- Location: Based at Grove House, St Albans but travel to all Rennie Grove Peace offices and external sites required

As part of the care home education team, this role will focus on supporting the delivery of a comprehensive outreach palliative End of Life Care (EOLC) education programme, and ongoing support to care homes, and home-care agencies within our locality.
Main duties of the job
- With support, delivery of prepared training to care homes either individually or with the Care Home EducationLead.
- To work alongside the Care Home and Outreach Education Lead providing ongoing support to identified carers and other professionals working in care homes and care agencies.
- Attend, where relevant, internal and external meetings, promoting the EOLC education programme.
- Acting as a Rennie Grove Peace ambassador within the community.

We provide palliative and end of life care support, including bereavement services, within Buckinghamshire and Hertfordshire and our patient services are available to adults, as well as patients transitioning into adult services (16 to 25 years). Bereavement support is offered for all ages to families of patients and those around them, as well as throughout the community, in schools and workplaces.

The Charity is now one of the larger UK hospice charities, reaching a GP population of 797,037 of whom 75% live in Hertfordshire and 25% in Buckinghamshire.

- Support in the promotion of an EOLC care home education programme and the wider support available through Rennie Grove Peace.
Operational
- Support with the recording and reporting of the monthly activity of the care home education programme.
- Support with the promotion of the service.
Clinical practice
- With support deliver an educational programme to care homes and home care agencies acting as a role model and always demonstrating Rennie Grove Peace values.
- Ensure own knowledge and skills are updated and maintained.
Professional/Personal
- Understand and comply with all Rennie Grove Peace policies and procedures.
- Maintain required organisational standards for all mandatory training.
- Keep up to date with current legislation and any training relevant to role.
- Demonstrate ongoing personal and professional development in accordance with annual Individual Progress Review (IPR) system.
- Ensure that information (in any form) relating to patients, staff, volunteers, customers and supporters is always protected and treated confidentially and complies with current data protection laws.
- Attend relevant meetings as appropriate.
