Job summary
The role of Health Care Assistant is
in place to provide end of life care to patients in their own homes in the
final weeks of life. You will work in the community and you will be travelling anywhere within the Rennie Grove Peace catchment area so a car and driving
licence will be essential.
Salary: £20,520 - £23,450 per annum (pro rata)
Hours: up to 44 hours per week.

Main duties of the job
Responsibilities include:
- Competently working alone, as required, with patients and their families in the home environment with remote support and guidance from the Rapid Personalised Care Service (RPCS) Management in adherence to lone worker policy and procedure.
- Working collaboratively with hospice and community partners.
- Acting as a Rennie Grove Peace Hospice Care ambassador, actively demonstrating our values and positively promoting the specialist care we provide.
