Job summary

The purpose of this role is to deliver care and support to patients, families, and carers at home for those identified as requiring care and support at end of life.

Main duties of the job

Competently working along, or with a registered health professional (nurse or paramedic), as required, with patients and their families in the home environment with support and guidance from qualified professionals. Respond to all calls and requests for patient visits, speaking with patients and relatives as required. To adhere to the lone worker policy and procedure. Working collaboratively with hospice and community partners. Acting as a Rennie Grove Peace ambassador within the community.
