Job summary

We are seeking to recruit a dynamic, organised and enthusiastic Practice Nurse to join our team.

This is an exciting opportunity for an enthusiastic and motivated individual to join our friendly team at Alma Medical Centre in Stockton. Applicants should have excellent communication skills and be passionate about high quality patient care.

Main duties of the job

As a successful post holder you will work as part of a multidisciplinary primary health care team delivering primary health care nursing within your competencies. Responsible for ensuring the delivery of safe and effective general nursing care to the practice population.

You will be responsible for the implementation of process of effective management of long term conditions, childhood immunisations and travel vaccination and screening programmes.

The suitable candidate will be a registered nurse preferably with experience working in General Practice desirably holding qualifications in asthma, COPD, diabetes, CHD, cervical screening.

Knowledge of the GMS contract, The Quality and Outcomes Framework, is a vital element of the post. Additionally, the successful candidate will be encouraged and supported to undertake any further training required to ensure appropriate role development.

Pay rate will be subject to qualifications and experience.

Alma Medical Centre provides excellent care to a population of approximately 11,600 patients. We are a training practice with 4 partners, salaried GPs, Advanced Nurse Practitioner and Primary Health Care support workers. We have a supportive and friendly administration and reception team.

Job description

Job responsibilities

You will undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs, Nurse Practitioners, and Practice Nurses, or independently, as appropriate for the need, according to practice policy, protocols, etc..

Main Duties

  • Assist GPs to help deliver effective implementation of management of patients with chronic diseases as detailed by the Quality and Outcomes framework, providing disease management clinics.
  • Undertake wound care / management / dressing techniques.
  • Provide an adult and childhood immunisation service, working to Patient Group Directions.
  • Health promotion, including breast awareness, cervical screening, and bowel screening.
  • Undertake cervical cytology.
  • Deliver travel vaccination and health advice.
  • Undertake male, female, and family planning assessments, giving advice and referring patients appropriately.
  • Undertake ECGs.
  • Offer guidance and support to healthcare assistants to improve standards of care, if needed.
  • Support the practice in achieving quality indicators and targets of the GMS contract and any relevant National Service Frameworks.

Additional Duties

  • Contribute to the development of policies / protocols / guidelines within the practice.
  • Support the implementation of evidence-based guidelines in conjunction with the practice nursing team.
  • Promote the continuing professional development of yourself and other nurses and HCAs within the practice, working alongside healthcare assistants to help develop their skills.

  • Undertake accredited training in specific chronic disease management, as / if required, e.g. diabetes, respiratory conditions, epilepsy, CHD, etc..
  • Contribute to the maintenance of the practice diseases registers.
  • Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
  • Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local referral procedures.
  • Ensure that current evidence-based approaches to patients are standard practice.
  • Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages, and report and treat sharps injuries.
  • Undertake first aid and management of emergencies, e.g. burns, haemorrhage, hypo- / hyperglycaemic emergencies, and asthma attacks.
  • Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques.
  • Provide professional advice / support to healthcare professionals within the Primary Health Care Team.
  • Following identified training needs, participate in training and facilitation of other practice nurses and healthcare assistants.
  • Provide mentorship and guidance for healthcare assistants.
  • Participate in partnership programmes of awareness raising, detection, and prevention of chronic diseases.
  • Maintain a level of expertise in specific chronic disease management (when appropriate) and other pertinent areas of healthcare and seek to develop new and improved skills.
