Job summary

We are looking for an enthusiastic and committed Clinical Pharmacist for a full time role. This would be to join our superb 5 partner and 2 salaried General Practice.

The role of the Clinical Pharmacist would be to support us with structured medication reviews, medicine optimisation, in house audits, provision of clinical support to duty team, consulting own patients and engagement in educational activities.

We pride ourselves on having a cohesive, friendly team of 2x Nurse Practitioners, 2x Practice Nurses and an HCA, they are complimented by a stable and highly capable administrative and reception team. We have a daily coffee break that you are encouraged to attend and has been invaluable for team cohesion.

Anticipated start date of Autumn 2023, but willingness to wait for the ideal candidate.

Main duties of the job

  • The post holder is a pharmacist, who acts within their professional boundaries, in collaboration with other pharmacists and technicians within the PCN
  • The post holder will work as part of a multi-disciplinary team in a patient-facing role.
  • The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy, especially for those with multiple co-morbidities
  • The post holder will provide primary support to the practice medicine teams and other practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription request, queries, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.
  • The post holder will advise on medicines optimisation and quality improvement and support some aspects of the quality and outcomes framework and enhanced services.
  • The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
  • Expected to enrol on the CPPE Primary Care Pharmacy Education Pathway in order to support the new Network Contract DES.

Job responsibilities

Primary Duties and Areas of Responsibility

  • Be responsible for the management of patients with chronic diseases by undertaking clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme)
  • Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities
  • Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services
  • Deliver a programme of structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care
  • Be involved in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload
  • Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system
  • Undertake a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation services
  • Be part of a professional clinical network and undertake appropriate clinical supervision

  • Have experience with / an awareness of the breadth of common long term conditions that are likely to be seen in a general medical practice
  • Have a minimum of two years post graduate experience in pharmacy
  • Be able to plan, manage, monitor, advise and review general pharmaceutical care programmes for patients across care areas, including disease states / long term conditions
  • Be accountable for delivering professional expertise and direct service provision
  • Use skills in a range of routine situations requiring analysis or comparison of a range of options
  • Recognise priorities when problem-solving and identify deviations from normal pattern, referring to GPs/NPs when appropriate
  • Be able to follow legal, ethical, professional, and organisational policies / procedures and codes of conduct
  • Involve patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines
