Job summary

As a Digital and Transformation Lead you will play a key role in the digital and transformation initiatives of the Primary Care Network. You would provide a central source of leadership and management to aid service transformation for our population.

The ideal candidate will need to be proactive, have a passion for Primary Care, and have a proven track record in delivering Primary Care improvements.

The remit of this role is broad and will require attention to detail in delivering high-quality day-to-day operations.

The Digital and Transformation Lead will work for Hartlepool Health Primary Care Network and the contract of employment will be held by McKenzie House Surgery. The post holder will be required to work across all of the three GP practices.

Main duties of the job

  • Lead and develop a strategic plan that includes a robust digital infrastructure
  • Clarify objectives and priorities for network delivery in line with operational capacity, financial parameters, estates and contract requirements
  • Proactively build relationships with system leaders to help influence proactive and new models of care
  • Use business intelligence tools to aid data-driven decisions
  • To ensure the PCN, and member practices, are conforming with national policy on digital priorities
  • To standardise, manage and monitor websites and social media accounts

As part of this support, we are hosting this job advert on behalf one of our member PCNs, Hartlepool Health Network.

Hartlepool Health consists of three Practices:

  • Headland Medical Centre,
  • McKenzie House
  • Wynyard Road Medical centre.

We meet monthly to discuss all aspects around the PCN and have a good working relationship providing joint Covid and Flu vaccination clinics along with a full team of PCN staff.

Job responsibilities

Line Management

  • The role will line manage all PCN ARRS staff

Strategic Planning

  • Lead and develop a strategic plan that includes a robust digital infrastructure
  • Clarify objectives and priorities for network delivery in line with operational capacity, financial parameters, estates, and contract requirements.

Operations

  • Provide oversight and accountability to the delivery of PCN contracts and specifications
  • Coordinate training and development of PCN and member practice staff
  • Identify and record risks and issues and develop contingency plans with Network Leads
  • Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented
  • Drive issues through to resolution, acting as a point of expertise
  • Implement network-based projects

Finance and Governance

  • Support the collation of financial information
  • Monitor and check the accuracy of the submissions for the DES and contractual requirements
  • Compile and present financial reports
  • Ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness

Digital and Transformation

  • Identify digital solutions to support the development, resilience and the future proofing of the PCN in line with local, regional and national needs.
  • Improve adoption of new technology and create data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff and patient experience.
  • Ensure the PCN complies with all Data Protection Regulations/GDPR when sharing data which may include reviewing or implementing DPIAs, Data Sharing Agreements, Assessments, etc.
  • Identify best practice and most efficient ways of practice systems working together for example EMIS and SystmOne
  • To support the management team in implementing service improvements identified by digital, public health, or other intelligence.

Relationship Management

  • Foster a culture of collaboration and promote learning to drive continuous improvement
  • Work as a flexible member of the PCN, providing support to other team members when necessary
  • Develop and deliver a communication plan using a wide variety of channels

Data and Population Health Management

  • To be the PCN lead on accessing national and ICB information platforms to ensure the PCN has the most up to date digital intelligence for example RAIDR
  • Effectively present and communicate data to practices and PCN staff to increase understanding of operational challenges

HR and Workforce Planning

  • Support recruitment, induction, training, performance reviews and HR elements of PCN staff management
  • Develop a coherent workforce plan to support recruitment
  • Facilitate actions to promote PCN staff retention

  • Any responsibility identified during the job. This is an evolving role, and the candidate will need to be flexible and adaptable.
