Join us as an End-of-Life Care Senior Nurse
and play a key role in managing our team and delivering high-quality patient
care. You will shape a positive working environment and foster professional
growth within your team.
Working closely with the Clinical Nurse Manager, you will
oversee Registered Nurses and Healthcare Assistants, ensuring patient-centred
care that aligns with clinical priorities. Your palliative care expertise will
help manage a defined caseload and encourage a proactive approach to
identifying patients nearing the end of life, ensuring timely transfers to
their preferred place of care.
This role is both challenging and fulfilling, offering the
opportunity to influence the care experience of patients and their families

Contract- Full-time (Rota's
will include days, nights and weekends)
What were looking for:

Registered Nurse on NMC Register
-
Experience in palliative/End of Life care
-
Be person-centred and compassionate when caring
for the person and those important to them
-
Experience of informal and formal
teaching
-
Up to date knowledge of current palliative care
issues
-
Ability to develop and evaluate care management
plans alongside other healthcare professionals that meets the complex needs of
the person, and those important to them
-
Proven understanding of the barriers and
enablers to good communication and how challenges can be overcome

Understands benefits of working with a
patient-centred philosophy of multidisciplinary team working

Understanding and support of the benefits of
clinical supervision and reflective practice

Flexibility to provide care in the area with the
most need
