Job summary
The purpose of the Support Nurse Post is to
provide high-quality nursing support as needed across all departments
supporting our work in Palliative Care, including evidence-based symptom
control, social and emotional care both to the patient and their support
network. The role will be responsible
for ensuring day to day clinical support across all Hospice clinical
departments as needed.
They
will support the team to ensure the delivery of the highest standards of care
to the patient. They will join key
clinical staff in the ongoing assessment, care and management of all Hospice
patients until they are discharged from the service.
Main duties of the job
- To provide direct day to day clinical support to Hospice
patients working collaboratively across all clinical departments.
- Put
quality at the heart of practice by delivering evidence based individualised
care through holistic needs assessment, care planning and evaluation of all
care interventions; determines and follows agreed care pathways.
- To
take the role of Key Worker for an allocated caseload demonstrating nursing
leadership in delivering episodes of care
- Assess
peoples information needs and provide relevant information to meet those needs
through a variety of solutions e.g. the use of information prescriptions;
supports people to understand and evaluate complex and new information, to
enable them to determine their own future.
- Promote
self-management, undertaking risk assessment to determine those patients who
can self-manage, following education, those who will need guided support to
self-manage and those who will require on-going, face to face support in coping
with life limiting illness and its treatment.
- Provide
clinical expertise, advice and support across service/agency boundaries; refers
to other professionals and sign-posts to other agencies as appropriate.
- Be
proactive with discharge planning to ensure a smooth transition for the
patient. Ensuring they are appropriately
discharged, signposted to other services if required ensuring they are aware of
how to contact us in the future.

are offered free of charge for adults in the community living with a life
shortening illness and their families, carers and friends. We employ a range of qualified and
