Great Oaks operates a Bank System to ensure
that we can meet the fluctuating demands on the system and has bank workers to
help to ensure that Great Oaks is able to offer a consistent, quality service
with appropriate staffing levels.
Main duties of the job
The purpose of the Hospice@Home Registered
Nurse is to provide high-quality Palliative Care including evidence-based
symptom control, clinical, physical, social, emotional, and spiritual care both
to the patient and their support network in the persons preferred place of
care.
The RGN will be responsible for care of the
patient and their support network and ensuring the highest standards of care
are delivered.
They
will be responsible for the ongoing assessment, care and management of the
person during the visit in line with standards set by NMC (Nursing and
Midwifery Council ), Mental Health Act (2005) and CQC (Care Quality Commission
)and Ambitions for Palliative and End of Life Care National Framework
2021-2026.

- Occupational Health screening is required for this role. There will be an expectation to be fit for role and carrying out light to moderate nursing duties such as manual handling.
Professional Knowledge
Essential
- Knowledge and experience in use of Shared Care Record for Last Few Days of Life
- Competent in managing medical devices including a syringe driver
- Ability to assess changes in condition and need for symptom control
- Be competent to give PRN medication as required
- Awareness of infection control issues
- Knowledge of Community Nursing
- Knowledge of keyworker role as a care delivery strategy
- Professional portfolio to demonstrate professional qualifications and experience
- Knowledge of evidence-based care
- Clear and concise documentation skills
- Awareness of Ambitions for Palliative and End of Life Care and the Gloucester end of Life Care Strategy
