We have an
exciting new opportunity to provide overnight non-clinical palliative care to our patients whose
wishes are to remain within their home environment.
This role will provide
essential support to our patients with life limiting
conditions, their families, and support networks.

Main duties of the job
The successful
candidate will work as Community Support Worker as part of a team contributing to the
provision of 24/7 End of Life Care. Responsibilities will not only be for
providing care, but for providing advice and support to patients and their
families within the community.
You will be trained to provide holistic care
and support from a non-clinical perspective and to effectively monitor and
escalate concerns to the community team and wider MDT to ensure patients
needs are met in a timely manner.
You will work with
patients who have life limiting illnesses and their support network adhering to
individually prepared care plans created to meet patients needs.
The successful
candidate will be expected to hold or achieve the Care Certificate
Qualification.
