Job summary

You will work as part of the Mountbatten Care at Home Team to provide care and support to our patients with life limiting illness, their families and carers within the Island Community.

Main duties of the job

To deliver direct care, treatment and emotional support for patients as required within set protocols/policies in accordance with the patients individual care plan.

To contribute to the assessment, implementation and monitoring of patients and carers, identifying physical and/or psychological problems and liaising with the appropriate health and social care team members.

Work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support.

To promote and maintain effective liaison with care team and other health and social agencies involved in the patients care.
