Job summary
You will be joining a passionate and committed team of Day One Caseworkers based in other Major Trauma Centres, driven to make a difference to the recovery of those impacted by catastrophic injury.
Day One Caseworkers deliver an on-site, bedside signposting and support service to identify and address any issues and concerns caused by the impact of major trauma. You will take referrals, complete initial assessments of need, support discharge planning and expertly navigate and signpost people in the aftermath of injury, including talking about and facilitating early access to legal support to aid rehabilitation. You will also play an active role in promoting the work of Day One within the hospital and wider community, including occasional fundraising and other events.
One post will be focused within the Major Trauma Centre in Salford and the other within community and hospital settings within the Northern Alliance Major Trauma Network.

Main duties of the job
- Work closely, and in partnership, with NHS clinical staff and relevant organisations, to address the needs of patients, and those closest to them, affected by catastrophic injury.
- Operate a case management approach to individuals, assessing and identifying needs, putting support in place including signposting, making referrals and direct support.
- Provide consistency in assessment of all patients, giving access to the earliest possible specialist legal advice which supports rehabilitation and NHS cost recovery.
- Maintain detailed case records, including activity and intervention, using a CRM system, in line with policies/ procedures/regulatory requirements.
- Implement processes and procedures to collect patient and family feedback and contribute to ongoing monitoring and evaluation.
- Build awareness of the role and charity's purpose through building strong and effective relationships across the regional major trauma network.
- Work closely with local authority, statutory and voluntary organisations.
- Support the patient discharge process in conjunction with leadership from NHS staff.
- Identify and support the recruitment and supervision of Day One volunteers.
- Support the roll out of our Peer Support Service.
- Work closely with the wider team on performance, quality, safety and effectiveness of all services.
- Work with our Fundraising and Communications team, supporting national and regional initiatives to raise awareness and fundraise.
- Willingness to undertake continuous development and training.

Day One is a young, ambitious national charity dedicated to supporting people impacted by serious or multiple injuries which could result in life changing consequences such as disability or death, referred to in the NHS as Major Trauma. Our Caseworkers work alongside NHS Clinicians, embedded as part of the team within Major Trauma Centres, to provide practical, emotional and financial support to adults and children as well as their families and loved ones. Inspired by patients for patients our vision is that no one has to piece life back together on their own after a catastrophic injury.
