Job summary

Pilgrims Hospices are looking for experiencedAdvanced Clinical Practitioners to join our team across our Hospices in East Kent, using their advanced practice skills to identify, assess, diagnosis, treat and manage patients with palliative needs.

Main duties of the job

The Advanced Clinical Practitioner will work across the patient pathway, utilising a full range of advanced practice nursing skills including: undertaking physical examination, requesting diagnostic interventions, identifying and implementing treatment plans, including appropriate prescribing and admitting and discharging patients to and from hospice care. The post holder will have important focus on supporting care professionals working in the wider health and social care to help ensure wherever possible, patients are cared for in a place of their choice.

Qualified as a Registered General Nurse, you will have palliative expertise and already be educated and or working at advanced level practice. Applications are also welcome from Registered General Nurses with palliative expertise, with a desire to pursue further training in a higher degree and associated advanced practice requirements in an Trainee Advanced Nurse Practitioner post.

  • Conversant in the use of Microsoft Office application.
  • Excellent verbal and written communication and negotiation skills.
  • Demonstrates positive leadership skills.
  • Strong time management skills Up to date knowledge of current palliative care issues.
  • Understanding and support of the benefits of clinical supervision and reflective practice.
  • Ability to work under pressure and to tight deadlines.

Experience

Essential

  • Capable and experienced working independently, able to plan and prioritise with advanced decision-making skills.
  • Management of change.
  • Partnership working across sectors.
  • Experience of formal and informal teaching.
  • Management of clinical issues related to Palliative Care.
  • Able to demonstrate previous successful experience of working at advanced practice level ideally in a palliative care setting.

Desirable

  • Experience in a specialist palliative care setting. Clinical leadership in a multi-professional service.
  • Research Experience Specialist Palliative Care Experience in a specialist palliative care setting.
  • Clinical leadership in a multi-professional service.

Qualifications

  • 1st Degree.
  • Current registration on parts 1 of the NMC register Independent Prescriber with scope of practice to allow the prescribing of high dose controlled drugs.
  • Completed advanced clinical examination skills training.
  • Masters in Palliative Care or closely related subject / Advanced Practice.

Desirable

  • Community Nurse Qualification.
  • NVQIV in Management or equivalent teaching qualification.
  • Independent prescriber who has prescribed in specialist palliative care.
  • Independent prescriber who has prescribed in specialist palliative care Completion of competency framework in palliative care [or willingness to complete].
