Based in Stockton (and opportunity to be based in Bishop Auckland Wed/Thurs/Fri)

An exciting opportunity has arisen to work as a Co-ordinator / Administrator with the Home Care Service. The role of this post is to provide accurate, effective, and timely organisational co-ordination and administration support to the Home Care Service, and Butterwick Hospice Care.

The successful candidate will work in partnership with patients and carers, undertaking co-ordination and administration duties within the agreed boundaries of their role. You will understand the importance of team work, and work closely with members of the health care team, and other members of Hospice staff.

Main duties of the job

It is essential that you have experience of working in a health or social care environment, and have excellent communication skills. You also need to have good attention to detail and be organised. You must be able to prioritise your workload, when there are competing tasks to be completed. You must be computer literate, and be able to produce simple reports.

You will predominantly work at the Stockton site, but may be asked to attend the Bishop Auckland site.

Confidentiality must be maintained at all times.

Access to a car is desirable but not essential.

Job responsibilities

Key Responsibilities

· To be responsible for the day to day co-ordination and administration of the Home Care Service that provides care for people who have a life-limiting illness, providing support and respite to patient carers and families.

· To receive and co-ordinate requests for care to the Home Care Service, ensuring the referral criteria has been met.

· To develop and maintain appropriate links with other care providers in order for the care to be arranged with the minimum of concern for the patient and family.

· To verify that risk assessments and care plans have been completed by the District Nurse, and are available in the patient’s home.

· To ensure that sufficient information is available to enable the care assistants to provide safe and effective care to the patient and family by following up with the District Nurse, prior to the visit taking place, until the required risk assessments and care plans are available in the patient’s home.

· To liaise with patients, family, care assistants, and other health care professionals, demonstrating excellent communication skills and appropriate sensitivity to the patient and family being supported by the Home Care Service.

· To produce duty rotas for care staff, ensuring adequate and appropriate skill mix at all times, and effective use of staffing resources to provide safe, high quality care to patients.

· Support the Day and Community Services Lead with the management of sickness and absence within the Home Care Service.

· To maintain accurate records and input data on to the iCare data collection system.

· To attend, contribute and take minutes at the team meeting, maintaining strict confidentiality at all times.

· Effectively liaise with members of staff providing care to patients, in order to ensure all activity data is captured and recorded accurately, clinical services are kept up to date and accurate reports are produced at required intervals.

· To ensure that referrals are accurately input on to the ‘iCare’ database and distributed to the relevant departments.

· To maintain appropriate manual and computerised systems as required including inputting data and maintain records within the patient database.

· To answer the telephone and deal with enquires, or directing the enquiry to an appropriate member of staff.

· Where appropriate to ensure accurate messages are taken, either in person or via the telephone and passed on in a timely and appropriate manner.

· To liaise with internal and external stakeholders and members of the public in and appropriate manner, both in person and on the telephone.

· To assist in providing administrative support to the Day and Community Services Lead in regards to patient and staff activity, statistics, and accurate monthly and quarterly reports. This to include reports on statutory and essential training compliance.

· To ensure that work is prioritised appropriately, high standards and quality of work are maintained at all times, and best practice is followed.

· To support the other members of the administration team when required, including undertaking essential administrative tasks for colleagues during holiday cover, thereby ensuring a continuous and reliable service across all areas of the Hospice.
