Job summary

The post holder will work alongside the PCNs Strategic Manager, the Clinical Director and the wider team to achieve success in line with the PCN Network Contract DES and the PCN priorities. Providing day to day operational support to the PCN, along with providing line management to the PCNs care coordinators and leading PCN projects.

Overview of the PCN:

South Warrington PCN is one of five PCNs within Warrington, Cheshire. We provide primary care services to 51,763 patients, across five GP practices. Stockton Heath Medical Centre, Latchford Medical Centre, Stretton Medical Centre, Brookfield Surgery and Lakeside Surgery

With the creation of the 2020/21 Network Direct Enhanced Services (DES) and the introduction of the NHS Additional Role Reimbursement (ARRS) to support primary care, the PCN has recruited staff under the ARRS to support and broaden our primary care team which allows us to provide better care for our patients. These roles include:

  • Paramedics
  • Mental Health Practitioners
  • Social Prescribing Link Workers
  • Health and Wellbeing Coaches
  • First Contact Physiotherapists
  • Care Coordinators

The PCN is headed by our committed Clinical Directors, Strategic Manager, Practice Managers and GP members.

Main duties of the job

To work with and support the PCNs Clinical Director, Strategic Manager, PCN members and their Teams to ensure effective delivery of the PCN DES and IIF requirements

Lead role for the PCNs Project Management Office (PMO)

Support equity of service provision across the Networks 5core practices

To provide line management to agreed PCN employed staff

Develop and maintain effective communication both with the ARRS staff and with all partner practices.

To provide support with project implementation and communications for new services.

Pro-actively identify and support implementation of areas of improvement.

Support the Digital Transformation Manager to develop/ maintain a patient voice forum, patient facing comms and social media presence for the PCN

Where required, take a lead role in project delivery. This could range to Program Management to supporting clinicians in the development of new pathways of care.

Help develop joint working and relationships across networks and stakeholders.

Deliver business plans.

Maintain excellent relationships across all core member Practices.

Minute meetings where required. Collating action points and circulating.

Supporting PCN members with all other reasonable administration requests.

Supporting and collating evaluation of PCN services to drive further improvements.

Support the financial delivery of PCN activities.

Help develop data sharing agreements for all PCN activities (when necessary)

About us

In addition to the PCN our Practices are also part of Quay Healthcare, which is a non-profit company owned by the GP Practices in Warrington. Quay Healthcare provide business and practical support to the PCNs and deliver additional healthcare services to our patients. The PCNs contract GP provider services through Quay Primary Healthcare CIC who are responsible for providing a number of services on behalf of the PCN; one of these services includes the management of our PCN ARRS staff, therefore this means that you will work alongsidemembers of the PCN and Practice staff on a daily basis, with Quay Healthcare as the lead employer.

Support the recruitment processes for new PCN staff, from advert to appointment, as required.

Arrange and oversee staff induction and training, ensuring staff have completed all training

The PCN is focused on building relationships with other local health and social care providers for the benefit of effective Primary Care service delivery to our patient population.

Demonstrate use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g., ICBs)

help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care

assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level;

provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; and support the coordination and delivery of MDTs
