Job summary
Clinical Pharmacists play an important role within Primary Care
Networks (PCN), working alongside pharmacy technicians, community pharmacists
and other members of the PCNs multi-disciplinary team. You will directly contribute
to improving patient care and supporting the efficient running of a new model
of general practice. You will become the practice expert in medicines
optimisation and have input into the following areas but not limited to:
undertaking structured medication reviews,
improving medicines optimisation and safety
supporting care homes
running practice clinics
You will be part of a group of PCN clinical pharmacists. You will
develop through team clinical supervision sessions and benefit from direct
mentoring with an experienced GP/ Clinical Lead. You will have the opportunity
to access and complete NHS Englands accredited training pathway that equips
you to practice and prescribe safely and effectively in a primary care setting
The post holder is expected to travel from GP practice
to GP practice and to other venues during fulfilment of their duties, therefore
a full clean driving license is essential, plus access to a reliable vehicle.
Experience is preferred but all applications will be considered as training is provided to all new starters and you will be enrolled on to an 18-month primary pathway.

Main duties of the job

To plan and organise own workload, including
audit and project work, and training sessions.
To record personally generated information and
maintain a database of information relating to the work done in the
practice(s). Personally generated information includes information and records
relating to audit and clinical work undertaken by the post holder, reference
notes relating to clinical/ technical information. The database includes
maintaining up-to-date, detailed records of all work done in practices for
which the post holder is accountable.
To maintain registration as a pharmacist and
comply with appropriate professional codes.
As appropriate to the post, to maintain and
develop professional competence and expertise, keep up to date with
medical/therapeutic evidence and opinion, and local and national service,
legislation and policy developments, agree objectives and a personal
development plan and participate in the appraisal process.
To attend local, regional and national meetings
of relevance.
To undertake any other duties commensurate with
the post holders grade as agreed with the post holders line manager.
All PCN Pharmacists are expected to prescribe as part of their role but if not already accredited you will be eligible for the fully funded Independent Prescribing course either during your pathway or upon completion of the pathway.
About us
Within Warrington there are five Primary Care Networks; four of the five networks are supported by our GP Federation known as Quay Primary HealthCare. The post holder will be employed by Quay Healthcare and will work across our Primary Care Networks and GP Practices.
Quay Primary Healthcare CIC is a Community Interest Company established
by Warrington GPs. Quay Healthcare supports flexible working and there may be
potential to do some of your hours remotely as part of a hybrid working model.
