Job summary

This is an exciting opportunity to join East Warrington Primary Care Network’s Multidisciplinary Team. We are looking for a dynamic individual with excellent interpersonal skills, who can work well in a team, whilst having the ability to work independently.

The successful candidate will be expected to grow and shape the role over the coming months by working effectively with a wider team of health care professionals to support implementation of the NHS Comprehensive Personalised Care Agenda.

Personalised care is one of the five major, practical changes to the NHS that will take place over the next few years, as set out in the NHS Long Term Plan; it represents a new relationship between people, professionals and the health and care system. It provides a positive shift in power and decision making that enables people to have a voice, to be heard and be connected to each other and their communities.

Main duties of the job

As a Health & Wellbeing Coach in Warrington, you will support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them, and help improve their health outcomes and quality of life.

You will be responsible for managing and prioritising your caseload according to the needs, and identify when a person needs to be referred to other health professionals or organisations. You will meet with patients both virtually and in person, and work across the three East Warrington practices (Padgate Medical Centre, Fearnhead Cross Medical Centre and Birchwood Medical Centre).

You must be flexible and be able to work during general practice core hours (when appropriate), which is Monday – Friday 8am-6.30pm.

Job description

Job responsibilities

· Embed coaching skills to motivate people over a period of sessions to self-identify their needs, set goals and develop the knowledge, skills, and confidence to manage their own health and wellbeing.

· Provide one-to-one and group health coaching support for people with one or more long-term conditions, based on what is important to them, with the aim of: improving people’s levels of ‘activation’, empowering people to manage their own health and improve their health outcomes.

• Manage and prioritise a caseload, in accordance with the needs, priorities and support required by individuals in the caseload. It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the health and wellbeing coach role – e.g. when there is a mental health need requiring the patient be referred to an appropriately qualified practitioner.

• Work as part of a multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self-Management, modelling the coaching approach in their work.

• Ensure that GPs, practice nurses, practice pharmacists and other members of the primary care team understand the health coach role, how to refer to them, and which patients may particularly benefit from health coaching.

• Support local health, social care and voluntary sector professionals to make appropriate referrals to the service. Promote and raise awareness of the health coaching service particularly to groups and communities that experience barriers to access.

• Attend and contribute to team, practice, and PCN meetings and events as required by the service.

• Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role.

• Participate in regular health coaching supervision and continual learning. This may include, but is not limited to, any or all the following: Regular contact with service supervisor refresher training sessions or Buddying with peers or Peer support sessions

Qualifications

Essential

  • Educated to GCSE level or equivalent.
  • GCSE Mathematics & English (C or above).
  • NVQ Level 3 Health Trainer qualification or other relevant professional academic qualification.
  • Smoking cessation training (or working towards completion).

Desirable

  • GP Referral Qualification (Wright Foundation or Gayton YMCA/Fit).
  • Relevant qualification in health and wellbeing/physical activity or nutrition.

Experience

Essential

  • Experience in a Physical Activity / Sports Development/ Health area within the public, voluntary or private sector.
  • Partnership working.
  • An understanding of the new NHS landscape including the relationships between individual practices, PCNs and the commissioners.
  • Detailed knowledge of the core concepts and principles of personalisation, patient activation, health behaviour change, self-efficacy, intrinsic motivation and assets-based approaches.
  • Experience with alcohol or drugs intervention.
  • Experience of working with groups.
  • Experience in health and nutrition work with patients.

Desirable

  • Broad knowledge of the NHS including general practice .
  • Good clinical system IT knowledge of EMIS/Systmone/Vision.
  • Access to own transport and ability to travel across the locality on a regular basis.
