Job summary

Adferiad provide a floating support service for individuals with housing-related support needs. Funded by Merthyr Tydfil County Councils Housing Support Grant. The service primariliy provides support for anyone with housing-related support needs enabling them to live independently in their own homes. The focus of the project is to develop or sustain an individuals capacity to live in their own accommodation and achieve increasing levels of independence. The project also includes supporting people with long term, enduring mental illnesses to live independently in their own homes.

You will work under the direction of the Service Manager & Senior to deliver support to the clients, providing high quality and effective housing related support service. Utilising person centred approaches to set goals, review and develop support plans that best meets the individuals needs, which focusses on each individuals strengths and sets goals of sustain their tenancies and live independently.

Main duties of the job

Provide a variety of person centred, tailored interventions that support individuals with complex needs to find sustainable accommodation and develop their independent living skills.

Working with service users in a proactive time and task specific manner on a variety of issues to impact positively on issues relating to housing, employment, training and education, offending behaviour and social isolation.

Manage a client caseload, and complete individual needs assessments, risk assessments, support plans and offer practical advice and advocacy to service users to ensure that an effective service is delivered to individuals.

Review progress, support plans & risk assessments & update accurately.

Build and maintain positive relationships with individuals with complex needs to ensure successful support, to address the persons accommodation and wider support needs.

Support service users to exercise informed choices about their lives where possible and to enhance their social skills and participate in their community.

Support service users to identify the areas of support needed and identify the best option of support based on ongoing needs as outlined in their individual Support Plans.

Work with service users to achieve their outcomes and the outcomes of the project, employing creative and innovative methods to meet these outcomes and, through their experience and resources, bring added value.

  • NVQ Level 2/3 in Health & Social Care, or other relevant qualification

Skills

Essential

  • Ability to prioritise workload and work under pressure to deadlines and changing demands on time.
  • Able to communicate clearly & effectively with a wide range of people and build and maintain constructive working positive relationships.
  • Good IT, report writing and note keeping skills, including maintaining all client records as required.
  • Ability to keywork and engage with a number of people who have a complex range of support needs.
  • Prepared to work on own and use own initiative, and able to work as part of a team.

Experience

  • Experience of working with vulnerable client groups, including individuals with enduring mental illnesses.
  • Experience of working in community based services.
  • Experience of working with individuals experiencing housing and housing related issues.
  • Experience of liaising with other specialist and non-specialist agencies.
