Job summary

We are an outstanding provider of expert palliative and end of life care and a prominent and respected charity at the heart of the local community. We cover our own inpatient beds (currently eight) see patients in their own homes, see patients as outpatients in our gym and see day patients when they arrive at our Wellbeing, Information and Support Hub (WISH) and also when they attend for blood transfusions, infusions in our nurse led Clinical Outpatient Service (OCS). We have developed a variety of groups where patients can attend with/without their carers to not only receive a high level of specialist interventions but peer support from others in a similar position. Our Specialist Palliative Physiotherapy Service assesses and treats those with an end of life/ palliative diagnosis whether it be cancer, end stage neurological or end stage organ failure. We offer individualised management plan involving the patient and families. The service is also for patients at the end of their cancer journey from diagnosis to end stage disease, offering preventative, restorative, supportive and palliative rehabilitation intervention. This may include those living beyond their cancer diagnosis left coping with the lasting effects of treatment.

In this post you will work across SW Essex and at a range of satellite venues. You will be a registered Physiotherapist, and whilst palliative experience would be helpful it is not essential. A drivers licence and use of own vehicle are essential.

Main duties of the job

To provide a Specialist Physiotherapy Service across South West Essex. To undertake specialist physiotherapy assessment and develop individualised physiotherapy interventions for patients presenting with problems in the field of palliative/end of life care, including carrying out ongoing review. To act as a resource for palliative care physiotherapy and to provide advice, education and support to the other members of the multi-professional team, and other healthcare professionals. To work within the Integrated Therapy Services (ITS), other Hospice and community services to provide support as appropriate e.g. well-being/life style management for survivors of cancer. To work with the HITS to develop clinics that can support effective dyspnoea, reduced balance, pain and symptom management, maintain mobility and promote life-style management and personal well-being. In conjunction with the Clinical Lead for OT and PT and Head of Integrated Therapy Services will support Physiotherapy Service, WISH, IPU and Hospice Community staff with regards to Physiotherapy assessment and management. To promote the key values of specialist palliative care for patients, families and carers, ensuring equitable access to physiotherapy service. To support and participate in the planning and ongoing service development of the physiotherapy service, working with the Service Lead, Head of Integrated Therapy Service, Director of Care etc.

1.1 To be responsible for caseload/workload and oversee that of the team and be an active participant in the wider multi-professional team, ensuring the holistic, physical, spiritual, emotional, psychological and social needs of patients are identified and addressed.

1.2 To plan, deliver and evaluate physiotherapy assessments and interventions to

address patients complex health and wellbeing needs, including one-to

one sessions and group therapy.

1.3 To run Out-Patient Clinics on a weekly basis.

1.4 To discuss progress of, and treatments for the patient with relatives, teaching

them therapy techniques if appropriate, and with the patients permission.

1.5 To keep clear records within the multi-disciplinary notes and on SystmOne, ensuring that both verbal and written communication is provided to a high standard.

1.6 To be responsible for the maintenance and safety of all physiotherapy equipment, including carrying out related risk assessments.

1.7 To be professionally and legally responsible and accountable for all aspects of clinical practice, including triage of new referrals and the management of

patients in your care.

1.8 To contribute to the improvement of the Physiotherapy Service. To participate effectively in multi-professional meetings and exchange of information, liaising with other professionals as required.

1.9 To represent the Physiotherapy Service, as appropriate, both internally and externally.

1.10 To contribute to local, regional and national networks/forums for community specialist palliative care as required.

1.11 To work proactively to maintain effective communication networks with all departments within the Hospice to maintain a seamless delivery of physiotherapy services.

1.12 To develop and adapt as needed long term condition management sessions.

1.13 In conjunction with other members of the multi-professional team, identify and

provide support to relatives and friends of patients, maintaining appropriate confidentiality at all times.

1.14 To actively promote and embed the use of End of Life Care tools within the role where appropriate.

1.15 To identify level of rehabilitation potential for individuals.

1.16 To develop and implement strategies to help cope with fatigue and insomnia and promote energy conservation.

1.17 To proactively identify symptoms/risk of malignant spinal cord compression and undertake specialist assessment and intervention for those with malignant cord compression.

1.18 To undertake mobility assessments to ensure safe discharge and re-admission avoidance, identifying cases where discharge from hospital/hospice may be complex.
