This is an exciting opportunity to join our Hospice Community Services team who ensure that the highest standard of holistic palliative care is delivered to patients and families in South West Essex.
You will have the time to care for patients and families and will truly deliver person centred tailored care; our team have supported people to plan weddings, helped with pets, ensured someone was pampered or supported them to watch a favourite film. Our Nurses value remembering the help they gave in final weeks and days
Working within Hospice Community Services means you are helping someone to understand what matters to them and live the best possible quality of life, when that life is at its most precious.
The Post holder is required to have a Registered Nurse Qualification, an appropriate recordable post basic Certificate/Statement of Competence, or equivalent level of skill acquired through experience.
The Post holder is required to have a minimum of two years post basic experience, one year of which should be in a specialist palliative care environment or related setting. Consideration will give to applicants who do not fulfil these criteria based on individual employment history, experience and competence.
Main duties of the job
The Postholder will participate as a member of the clinical nursing team
in the maintenance and development of agreed standards. He/She will be expected to carry out all
relevant forms of care to a high level and will be designated to regularly take
charge in the absence of the Senior Nurse Coordinator/Clinical Lead/Nurse
Manager. He/She will supervise and
direct junior members of staff and participate in teaching of other staff, as
appropriate, including basic and/or post basic students. He/She will be required to actively
contribute to the development and well-being of colleagues

1 Ensure the physical, spiritual, emotional, psychological, social and
cultural needs of patients are identified and attended to, providing and
maintaining the highest possible level of nursing care.
1.2 Participate in the telephone and face to face
triage assessment planning and review of patients’ needs. Devising
individualised care plans and ongoing reviews to ensure care plans are
individual/appropriate for patients, encouraging discussion and participation
by all team members including the Multi-Disciplinary Team giving and
receiving patient-centred handovers.
1.3 Active involvement within the Multi-Disciplinary
Team Meetings. Ensuring patients are identified and presented for review in a
clear and concise manner
1.4 As part of patient reviews, identify and provide
support to formal carers, relatives and friends of patients/residents, maintaining confidentiality at all times and
ensuring good relationships with patients’ relatives by providing frequent
contact and availability. Liaising with support services.
1.5 Ensure that all medication, treatments,
therapies and care interventions are clinically reviewed and administered in
accordance with Hospice Policies, Procedures and Standards.
1.6 To ensure the safe custody, storage and control
and administration of all drugs in line with national standards and the Hospice
Medicines Management Policy maintaining awareness of the Medicines Management
meeting and role of the Accountable Officer for Medicines Management.
1.7 Ensure that the highest standards of nursing
care are maintained. Assist and supervise
other members of the nursing team as appropriate, ensuring that all staff work
to and achieve an acceptable safe standard and the highest quality care.
1.8 To work
closely with all members of St. Luke’s Hospice Community Services to ensure
rapid discharges are supported effectively, and unwanted and inappropriate
admissions to acute care are avoided. To undertake care reviews as required

1.10 Ensure that verbal and written communication is provided to a high
standard and that
documentation is
completed accurately on SystmOne electronic records. Sensitive information
