General Practitioner with Special Interest in Frailty

Job summary

We are looking to recruit both full and part time General Practitioners with Special Interest in Frailty to join our Whole Systems Integrate Care team. This team is responsible for working with a range of partner organisations such as acute trusts, mental health professionals, local authorities and community services to ensure the appropriate level of care for service users and patients across the borough.

Main duties of the job

Key Responsibilities are:

  • Provide planned care and support as required, including medically prescribed clinical treatments, personal care, end of life care and wider health and social care as needed;
  • Work with a wide range of professionals, including GPs, social workers, community nurses, physiotherapists and occupational therapists and acute frailty teams to encourage identification of patients and referral to the service.
  • Accept referrals from all health and care staff who approach the service, including GPs, social workers and community nurses;
  • Work with health and care professionals to identify individuals who may benefit from support;
  • Work with Internal and external stakeholders to deliver a seamless transition between the hospital and the community;
  • Use a frailty trigger tool to identify the needs of patients referred to the service and the level of support needed;
  • Ensure patients have an anticipatory care plan in place;

Dr David Lubel, Consultant Geriatrician, leads our team. The team, supported by GPs with special interest in Frailty and Enhanced Nurse Practitioners, and other health and social care professionals main aim is to care for our housebound patients with complex health and social issues, providing continuity of care and reduce the number of unnecessary non elective hospital admissions and LAS callouts in Harrow.

If you want to lead the clinical team in making a positive change to patients and carers quality of life, you could be the right person for this role.

Full Registration with the GMC

Post registration primary care experience

Relevant clinical experience with older adult patients

  • Be able to effectively communicate with patients in a way which respects their privacy, dignity, needs and beliefs
  • Be a UK Registered Nurse with significant senior post registration experience
  • Have excellent communication skills and clear concise writing record keeping skills
  • A full UK drivers license and access to own vehicle
