Main duties of the job
We have an exciting opening for an Admissions and Discharge Coordinator to support the IPU. This role would be perfect for a Registered Healthcare Professional interested in coordinating referrals and admissions and setting up a robust discharge process to enable timely discharges of our patients. You will be the first point of contact for the hospice referrals and able to carry out initial clinical triage. Responsible for actively leading, directing, supporting and facilitating the identification of EOL care for patients wishing to return home as their preferred place of care. Coordinate all discharges to patients own home or care home as appropriate. The post holder must work in accordance with their registering bodies' code of professional conduct and Sue Ryder policies and procedures. You will also participate in the senior nurse on call rota.
