About the role
We have an exciting new vacancy for an Admissions and Discharge Coordinator to support the IPU. This role would be perfect for a Registered Healthcare Professional interested in coordinating referrals and admissions and setting up a robust discharge process to enable timely discharges of our patients.
You will be the first point of contact for the hospice referrals and able to carry out initial clinical triage. Responsible for actively leading, directing, supporting and facilitating the identification of EOL care for patients wishing to return home as their preferred place of care. Coordinate all discharges to patients own home or care home as appropriate. The post holder must work in accordance with their registering bodies' code of professional conduct and Sue Ryder policies and procedures. You will also participate in the senior nurse on call rota.
Main duties of the job
About you:
You will be an Registered Healthcare Professional and will have:
Experience in working with the admission and discharge processes of patients within an inpatient setting
Palliative care experience and knowledge
Knowledge of continuing healthcare (CHC)
Excellent and effective communication and interpersonal skills including having difficult conservations with a wide range of patients, relatives and other professionals
A prompt response to problems, including identifying and implementing solutions
An ability to actively support or participate in a multi-disciplinary team
Ability to work on own initiative
Awareness of relevant legislation including the Mental Capacity Act and Safeguarding
Computer literate
Evidence of continuing professional development
Willing and able to have a flexible approach to working patterns
