About us:
Sue Ryder St John's Hospice is in Moggerhanger in Bedfordshire, and we provide expert care for life-limiting conditions. Were here to help patients and their loved ones, with everything from pain and symptom management to emotional and practical support.
St Johns Palliative Care Hub provides palliative care 24 hours a day, seven days a week for individuals with a life-limiting condition who choose to receive care at home.
The Palliative Care Hub works together with local care providers, GPs, district nurses, specialist nurses, the ambulance service and other health and social care professionals to coordinate and provide the best possible care.
Main duties of the job

This is an excellent opportunity for an experienced nurse to join our busy but very friendly community team at St Johns Hospice.
If you are an enthusiastic and committed NMC registered nurse who is interested in a rewarding career within palliative care we'd like to hear from you!
This is a non-patient facing role where you will operate our 24 hour telephone support line working alongside another Registered Nurse or Nursing Assistant.
You will provide palliative care support and advice to our Patients on the Palliative Care Hub register, their families and other care professionals whilst signposting to other agencies as and when required. Therefore you will require excellent Assessment, planning, implementation and evaluation skills to ensure our patients receive high standards of personalised care.
You will also be required to support and lead the visiting Nursing Assistants and be responsible for the allocation of the teams visits.
Shifts are either 7.45am - 8.15pm or 8pm - 8am (with a 30 minute unpaid break).
