We are seeking aClinical Fellowto provide holistic and high-quality care to patients across inpatient, outpatient, and community settings. You will play a vital role in supporting patients and their families, working alongside a skilled and passionate team.

Main duties of the job
- Complete initial
assessments and treatment plans for inpatients
- Provide daily patient
reviews and symptom management
- Attend multidisciplinary
meetings and consultant ward rounds
- Support community and
outpatient care with supervision as needed
- Communicate with GPs
regarding patient treatment and discharge
- Certify deaths and liaise
with the Medical Examiner or Coroner
- Maintain clear, accurate
documentation
- Recognise personal limits
and seek appropriate support

St Lukes Hospice provides specialist end-of-life and palliative care to people in Harrow and Brent. Founded by local communities in 1984, we offer free, expert care through our Inpatient Unit (IPU), home care, outpatient clinics, and our 24-hour Palliative Helpline (Pall24). Two-thirds of our care takes place at home, respecting patients' preferences. Our holistic approach includes symptom management, patient and family support, complementary therapies, and bereavement care. We work collaboratively with NHS partners and community services to ensure compassionate, dignified care. Additionally, we provide training to healthcare professionals to enhance end-of-life care skills. Our services are made possible through the generous support of our community, with only 25% funded by the NHS.
