Job summary
The East Lancashire Alliance is currently looking to recruit a Full-time Nurse Associate on behalf of the Rossendale PCN.
Please note: This role will be based at various locations across Rossendale, working
as part of the TNA / NA visiting service for Rossendale PCN. Own car and Full Driving License is essential.
Main duties of the job
The Nurse Associate will:

Be responsible for delivering evidence based, high quality,
patient centred, compassionate care every time.

Work as part of a designated clinical and care team delivering care that focuses
on the direct needs of the individual

The responsibility
of the postholder is to participate in:

The planning
and delivery of Enhanced Health in Care Homes model of
proactive care that is centred on the needs of individual residents, their
families and care home staff

Chronic
Disease Reviews with housebound patients

Sessions within a practice setting

The Nurse Associate will:

Provide and monitor
care
- Develop Understanding of all elements of the
nursing process and be able to assist the lead nurse in the on-going
assessment, planning, management, and evaluation of care
- Support individuals with all aspects of care
including daily living, providing person-centred care and promoting
health and independence through awareness raising and
care navigation to other professionals as appropriate
- Perform and record clinical observations
including blood pressure, temperature,
respirations, pulse, and NEWS
