Job summary

An exciting opportunity has become available for two experienced Healthcare Assistant (HCA) to work with the 9 GP practices in Rossendale as part of the Rossendale Primary Care Network (PCN) plan to improve the health outcomes of the Rossendale population in preventative health care via the NHS Health check in community settings and improving the health outcomes in those patients diagnosed with diabetes using the 9 key processes in practice. 

This is a 12 month fixed term contract, the hours are 30 across 5 days

Main duties of the job

Working under the direct supervision of the nursing team and strictly in accordance with specific practice and PCN guidelines and protocols, the healthcare assistant will assist the clinical team in the provision and delivery of prescribed programmes of patient care

Job responsibilities

Clinical Practice 

The post holder will make professional decisions, providing safe, evidence-based, cost-effective, individualised patient care. 

To include but not limited to; 

- NHS Health Checks 

-Phlebotomy, Blood Pressure readings

-Height, Weight, BMI,

- Assess, plan, develop, implement, and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being 

- Support patients to adopt health promotion strategies and encourage principles of self-care 

- Manage QRisk following protocols. 

- Weight Management & Advice and referrals to health trainers, social prescribers, weight management service, drug and alcohol and stop smoking services. 

Diabetic Reviews 

- Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with Diabetes 

- Phlebotomy, 

- Blood pressure readings 

- Height, Weight, BMI, Waist circumference, 

- Foot examinations 

- Handling sample for Urine ACR 

- Support patients to adopt health promotion strategies and encourage principles of self-care 

- Weight Management & Advice and referrals to health trainers, social prescribers, weight management service, drug and alcohol and stop smoking services. 

Other duties 

- Infection control

- Helping to raise awareness of health and well-being and how it can be promoted 

- Willing to adapt and develop as per the needs of the PCN additional duties as requested by the clinicians or managers.

As this practitioner will be working across a number of practices and community settings it is important that you are able to prioritise and contribute to the delivery of high standards of care to patients, families and their carers in a safe and supportive environment. As part of the PCN clinical team you will also be asked to assist in developing and maintaining services provided to patients and contribute to the safe, efficient, and smooth running of the clinical area/department. The post holder will be seeing patients that are presenting at one of the PCNs practices by appointment. This role will be involved in the patient journey from initial history taking, clinical assessment, and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. In addition, the post holder will demonstrate critical thinking in the clinical decision making process. They will be expected to work collaboratively within the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing leadership as required.
