Job responsibilities

To undertake work in line with PCN directed priorities.

Support Quality and Outcome Frameworks, other DES/LES specifications, PCN DES targets and other locally agreed PCN targets.

Provide coordination and navigation for people and their carers across health and care services, aligning work to reflect the PCN needs for pharmacy service provision.

To liaise with PCN Pharmacist on service delivery, agreeing annual targets to reflect PCN DES and/or clinical targets as determined locally and agreed by PCN Clinical Director.

To support the design and ongoing implementation of structured medication review and care homes review services.

To liaise with care home staff across the network, and other relevant colleagues, in the coordination of a care homes pharmacist visitation service.

To encourage and arrange the provision of care for housebound patients, those who are lost to follow up, and to support the inequalities work of the PCN.

To develop and run clinical searches, with support from the PCN pharmacist, to identify patients who would benefit from pharmacist intervention and/or review.

Liaise with the care co-ordinators working at other network practices to share and develop best practice at PCN level.

Proactively identify and work with a cohort of patients who are high-risk for medication related harm, or have polypharmacy and/or multimorbidity, to ensure they are engaged with primary care and accessing review and follow-up services.

Ensure regular and consistent communication with the PCN pharmacist regarding patient progress and any complications or guidance.

Liaise with PCN pharmacist/other relevant clinician to ensure tasks are completed and care delivered in reasonable timeframes.

Act as the first port of call for caseload patients in relation to their care across the network practices.

Directly liaise with multi agencies/care providers to coordinate care for patients.

Co-ordinate referrals as required by PCN pharmacist across the network.

Carry out searches on EMIS to identify patients for recall and ensure invitations are sent in a timely manner via telephone, text or letter and ensure this is read coded to patients medical record.

To support patient/carer contact roles and collate patient and carer feedback on their experiences of PCN services.

To help people to manage their needs, answer their queries and support them.

To develop the role of Pharmacy Care Coordinator within the practices.

To maintain and develop engagement with all practice staff and encourage best practice.

To support national screening programmes to increase local uptake, where appropriate or relevant for workplan.

To complete training requirements as specified.

Responsibility for administration

Computer data entry/data allocation and collation, processing and recording information in accordance with PCN procedures.

Any other duties commensurate with this position.

Duties will vary from time to time under the direction of the PCN pharmacist / PCN clinical director on current and evolving PCN workload.

Person Specification

Qualifications

Essential

• Good standard of Education
• Enrolled/in progress/qualified from appropriate training as set out by the Personalised Care Institute (PCI) or willing to undertake.

Desirable

• Has safeguarding knowledge either level 2 or the equivalent experience

• Communicate sensitive information concerning patients medicines to patients and carers, overcoming barriers to understanding and communication with patients who have physical, mental, social, learning, language, hearing or sight problems
• To deliver outcomes based on set targets
• Liaise with other colleagues to promote the success of the PCN and its Partner organisations in primary care
• Work to perpetuate a culture that is transparent and open
• Could collate data and use tools to demonstrate the impact of services
• Recognises and works within limits of competence, seeking advice when needed
• Awareness of and has well-developed IT skills to facilitate quick and confident learning of new systems to support the management of patients in a primary care setting (including Microsoft packages)

• Demonstrable understanding of the personalised care approach

• Respond to medication related queries from healthcare professionals and patients referring to the pharmacist or other team members where appropriate
• Experience of working directly in a care coordinator role, in adult health, social care, learning support or public health/health improvements
• Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a voluntary or paid capacity)
• Experience of working in multidisciplinary professional team environments
• Experience or training in personalised care and support planning
