Main duties of the job

On a day-to-day basis, you will be working alongside the general practice team across the PCN helping patients to get the best from their medicines whilst operating within clear protocols and guidelines and supporting our clinical pharmacist to reduce our clinical risk and waste. Working side by side with the reception/care navigation team, the repeat prescribing service will run smoothly as you be instrumental in resolving their prescribing queries.

About us

The Roxton Practice has provided medical services to the people of Immingham and the surrounding area for more than 60 years. The first home for the practice in Immingham was a small house in Manby Road, serving the population of Immingham. The Roxton Practice then moved to Worsley Road in approximately 1960 until 2004 when they moved into the Pilgrim Primary Centre in Immingham. More recently Roxton also provides services for our Grimsby residents at Weelsby View Health Centre as well as operating from The Health Centre in Keelby

Job Purpose

To work within your professional boundaries as a registered Pharmacy Technician as part of the multi-disciplinary team within general practice. Your role is patient facing to support medicines optimisation and improved prescribing practice, patient care and deliver the practice prescribing agenda.

Job Summary

You will help patients to get the best from their medicines, operating within clear protocols and guidance, reducing clinical risk and waste.

You will facilitate the smooth running of the repeat prescribing service with support from senior colleagues on complex matters and collaboration with the reception / care navigation team to resolve straightforward prescribing queries.

Key Areas of Responsibility

Through patient facing/supporting roles ensure effective medicines use by shared decision-making with patients.

Work with the Practice multi-disciplinary team to support medicine optimisation tasks through effective medicine administration (e.g., checking inhaler technique), supporting medication reviews, medicines reconciliation, efficient ordering and return process and reduction of waste.

Support medication reviews and reconciliation for new care home patients and synchronise medicines for patient transfers between care settings.

Provide expertise to address both the public health and social care needs of patients where competent to do so on aspects such as lifestyle advice, service information and help tackle health inequalities.

Supervise practice reception teams in sorting general prescription requests to allow GPs and clinical pharmacists to review the more clinically complex requests.

Take a central role in the clinical aspects of share care protocols and liaising with specialist pharmacists for more complex patients.

Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.

Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.

Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings, through clinical audits (e.g., use of antibiotics), quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.

Provide leadership for medicines optimisation systems across the practices with a range of services to get the best value from medicines by encouraging and implementing Electronic Prescriptions, safe repeat prescribing systems, and timely monitoring and management of high-risk medicines.

Provide training and support on the legal, safe, and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

Develop relationships with other pharmacy technicians, pharmacists, and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care, and mental health.

Management of workload and personal development

Act on own initiative in prioritising and managing workload, in respect of clinical, managerial, and administrative areas, as necessary.

Maintain own personal and professional development as agreed at your annual review.

Essential

  • Enrolled in, undertaking, or qualified from, an approved pathway such as, (PCPEP) or (MOCH)
  • Registered to the General Pharmaceutical Council (GPhC) as a Pharmacy Technician
  • BTEC/NVQ level 3 or equivalent in pharmaceutical development
  • Evidence of continued professional development (CPD)
  • Relevant advanced theoretical and practical knowledge of Primary Care Networks, General Practice, and evidence-based medicine.
  • Awareness of GP budget-management and funding systems to enable GP clinical pharmacist services
  • Pharmacy law, ethics, and current legislation
  • An appreciation of primary care prescribing, concepts of rational prescribing and strategies for the improvement of prescribing
  • An excellent practical understanding of data protection and confidentiality
