Job summary

Main duties of the job

As an ANP, you will be a degree qualified, experienced clinician with previous exposure to Primary Care environments. The role will provide acute, same day care to patients. This will include face to face appointments, telephone triage and home visits as required. Although part of a larger team of GPs, PAs, Practice Nurses and support staff, you will be confident working autonomously.

Assess, diagnose, plan and implement treatment for patients Complete or instigate onward referrals where appropriate Diagnose and manage acute and chronic conditions

Prescribe and review medication in accordance with national protocols

Undertake annual reviews

Communicate compassionately and manage sensitive or difficult conversations with patients, relatives and carers

Actively implement safeguarding protocols

Contribute to local and national targets eg QAF, IIF, Prescribing Incentive Scheme

Keep up to date and accurate records using S1 using templates and READ codes

Attend and participate in Practice and PCN meetings as required

Develop and maintain a personal learning plan

Ensure clinical practice is safe and remains within boundaries of competence

HASP PCN is a collaboration of five GP Practices in the Hull area. Serving approx. 35,000 patients we are looking for two Advanced Nurse Practitioners to join our team. We are a forward thinking organisation, working in line with the national DES with a high focus on Health Inequalities.

Job responsibilities

Key Areas of Responsibility

To work alongside the multi-disciplinary team across the PCN to assess and triage patients, including same day triage.

Provide definitive treatment such as the prescription of medications in accordance with guidelines (locally and nationally) and make referrals to other members of the primary care team.

Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.

Undertake complex decision making to inform the diagnoses, investigation, and complete the management of episodes of care within a broad scope of practice.

Proactively take a personalised care approach and population centred care approach to enable shared decision making with the presenting patient.

Building capacity through effective leadership

Provide clinical supervision to others in clinical roles within primary care such as first contact practitioners and personalised care roles.

Risk Management and Quality Assurance

Ensure robust clinical governance is in place for your practice which includes regular audit and evaluation, supervision, and CPD.

Management of workload and personal development

Act on own initiative in prioritising and managing workload, delegating tasks appropriately and referring to designated MDT/Board Members in respect of clinical, managerial, and administrative areas, as necessary.

Maintain own personal and professional development as agreed at your annual review.
