Job summary

We are looking for a suitably qualified and experienced Dietician under NHS England’s Additional Roles Reimbursement Scheme (ARRS) to work in our expanding multidisciplinary team (MDT) on behalf of Bridlington Primary Care Network member practices.

Main duties of the job

As an integral part of the multi-disciplinary team working across our PCN, you will provide a dietetic service, giving expert knowledge in the management of patients’ dietary issues and conditions. We will also look to you to drive the development of the dietetic service forward locally.

You will offer interventions for the patient from initial clinical assessment, diagnosis, treatment, and evaluation of their care. Working collaboratively to meet patients’ needs, you will support the development of policy and procedures with your evidence-based leadership approach.

About us

Bridlington (PCN) is a network of six GP Practices namely: Field House Surgery; Manor House Surgery; Practice One, Two, Three and Wolds View Surgery. We are one of seven PCNs across the East Riding and cover a registered population of over 41,794 patients.

Job Purpose

As an integral part of the multi-disciplinary team working across our PCN, you will provide a dietetic service, giving expert knowledge in the management of patients’ dietary issues and conditions. We will also look to you to drive the development of the dietetic service forward locally.

Job Summary

You will offer interventions for the patient from initial clinical assessment, diagnosis, treatment, and evaluation of their care. Working collaboratively to meet patients’ needs, you will support the development of policy and procedures with your evidence-based leadership approach.

Key Areas of Responsibility

· Independently manage caseloads of complex patients with a broad range of conditions, using an evidence-based approach to assess, plan, implement and evaluate.

· Plan, organise and undertake nutritional assessments.

· Share knowledge and information to inform patients with diet-related disorders so they can see how they can improve their health and prevent disease through the adoption of healthier eating and drinking habits.

· Provide support to patients from early life to end of life in a range of settings in the community including nurseries, patient homes and care homes.

· Offer highly specialised advice and education to care homes and other clinical settings for patients with multiple conditions to determine the appropriate nutritional intervention/treatment.

· Implement proactive support mechanisms and techniques to encourage behavioural change to optimise health outcomes for the patient.

· Having the necessary prescribing qualification, where necessary, recommend and authorise changes to prescribed products, ensuring appropriate monitoring is in place.

· Develop, implement, and evaluate, continually improving standards and performance of nutrition support services across the PCN, in collaboration with community and secondary care.

· Support the smooth transition of patients from elsewhere back into primary care to facilitate the continuation of their diet plan.

· Make informed recommendations to PCN colleagues regarding changes to medications for patients’ nutritional management.

· Ensure the prescribing of dietetic related products is both clinically and cost effective.

· Refer patients onwards when appropriate.

· Participate in peer networking/review and engagement in evidence-based practice, supporting regional and national research and audit, to evaluate and improve the effectiveness of the dietetic service.

· Embed evidence-based findings into pathway developments and practice locally.

· Monitor and track risks and issues facilitating escalation and resolution.

· Ensure robust clinical governance is in place for your practice which includes regular audit and evaluation, supervision, and CPD.

· Be involved in the development and implementation of relevant policies, formulary updates and clinical management plans.

Management of workload and personal development

Act on own initiative in prioritising and managing workload, in respect of clinical, managerial, and administrative areas, as necessary.

Maintain own personal and professional development as agreed at your annual review.

  • Has a BSc in Dietetics under a training programme approved by the British Dietetic Association (BDA)
  • Registered member of the Health and Care Professional Council (HCPC)

Desirable

  • Pre-reg MSc Dietetics under a training programme approved by the British Dietetic Association (BDA)
  • Completed or working towards a supplementary prescribing qualification

Experience

Essential

  • Working well with others to deliver goals
  • Being motivated to deliver excellent services to support Primary Care in our region
  • Significant experience in applying the dietetic process to include dietetic assessment, interpretation, individual care planning, motivation, monitoring and evaluation of highly specialised dietetic treatment.
