Job summary

We are looking for a suitably qualified and experienced Pharmacy Technician under NHS Englands Additional Roles Reimbursement Scheme (ARRS) to work in our expanding multidisciplinary team (MDT) on behalf of North Care Network member practices.

Main duties of the job

This is a new and developing role; you should hold, be in enrolled in, undertaking, or qualified from, an approved pathway such as, (PCPEP) or (MOCH) and Registered to the General Pharmaceutical Council (GPhC) as a Pharmacy Technician.

You will be working with the general practice teams across the PCN to support medicines optimisation and improved prescribing practice, patient care and deliver the Directed Enhanced Service PCN prescribing agenda. You will be involved in MDT review and supporting Care Plans.

About us

North Care Network is a recently formed Primary Care Network (PCN). We consist of 3 like-minded North Lincolnshire practices based in Barton-upon-Humber, Brigg, Winterton and the surrounding villages. We are responsible for the care of around 33,000 patients.

The successful candidate will work independently at network and practice level, but also as part of a supportive network team, to deliver the safe and effective use of medicines by applying their pharmaceutical skills to a variety of tasks within the medicine's optimisation agenda. You will have access to pharmacists for support and supervision.

All of our member practices run dispensaries and at times you may be asked to support and guide the dispensing staff to ensure safety and compliance with medicines management.

Job Purpose

To work within your professional boundaries as a registered Pharmacy Technician as part of the multi-disciplinary team within general practice and the Primary Care Network. Your role is patient facing to support medicines optimisation and improved prescribing practice, patient care and deliver the Directed Enhanced Service PCN prescribing agenda.

Job Summary

You will help patients to get the best from their medicines, operating within clear protocols and guidance, reducing clinical risk and waste.

You will facilitate the smooth running of the repeat prescribing service with support from senior colleagues on complex matters and collaboration with the reception / care navigation team to resolve straightforward prescribing queries.

Key Areas of Responsibility

· Through patient facing/supporting roles ensure effective medicines use by shared decision-making with patients.

· Work with the PCN multi-disciplinary team to support medicine optimisation tasks through effective medicine administration (e.g., checking inhaler technique), supporting medication reviews, medicines reconciliation, efficient ordering and return process and reduction of waste.

· Support medication reviews and reconciliation for new care home patients and synchronise medicines for patient transfers between care settings.

· Provide expertise to address both the public health and social care needs of patients where competent to do so on aspects such as lifestyle advice, service information and help tackle health inequalities.

· Supervise practice reception teams in sorting general prescription requests to allow GPs and clinical pharmacists to review the more clinically complex requests.

· Take a central role in the clinical aspects of share care protocols and liaising with specialist pharmacists for more complex patients.

· Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.

· Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.

· Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings, through clinical audits (e.g., use of antibiotics), quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.

· Provide leadership for medicines optimisation systems across PCNs, supporting practices with a range of services to get the best value from medicines by encouraging and implementing Electronic Prescriptions, safe repeat prescribing systems, and timely monitoring and management of high-risk medicines.

· Provide training and support on the legal, safe, and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

· Develop relationships with other pharmacy technicians, pharmacists, and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care, and mental health.
