Job summary

We have 4 vacancies available in this role; for a fixed term period until March 2026.

This service will be a prominent part of the WorkWell Pilot in Stockport. The WorkWell Pilot is commissioned to run for 18 months. This service aims to provide disabled people and people with health conditions the opportunity to access an early intervention work and health assessment service with low-intensity holistic support for their health-related barriers to employment. It also aims to provide a single joined-up view of and gateway into the services that are available locally to tackle specific needs. This could include healthcare professionals, community sector services, health promotion programmes, more intensive employment support and much more.

Main duties of the job

The post holders will build links with existing local employment and skills support, community-based health provisions, social prescribing service, mental wellbeing resources and GP / Primary Care.

The post holders will have responsibility for proactively supporting people in employment or recently unemployed to support their health and employment needs, be that accessing health services or finding a new job.

You will be responsible for carrying out initial assessments, developing action plans and supporting the individuals to help them reach their goals for each person who accesses the service.

About us

Viaduct Care CIC is the company structure for Stockports GP Federation and represents all of its local GP Practices. Covering a patient population of circa 300,000 practices are split into 6 PCNs with each serving a population of around 30,000-50,000 patients.

Viaduct Care represents the collective voice and interests of its member practices and as a key stakeholder in Stockport Together aims to influence and support the design and delivery of major service and system changes by being a strong and effective partner with other major service providers.

Job responsibilities

The role will provide the following support to participants

  • An initial assessment (which is evidence-based, person-centred, and holistic) of barriers to and opportunities for employment;
  • Return-to-work/thrive-in-work planning, with clear objectives that address physical, psychological and social needs;
  • Personalised work and health support with follow-up as required, including ongoing support in the form of locally determined, low intensity appointments to take stock of progress and recommend further actions and activities. As these are non-clinical roles, they will not include clinical advice.
  • Employer liaison - if the participant consents, the employer can be contacted to share the work plan and provide advice;
  • Information and advice on reasonable adjustments, how to seek support to arrange adjustments and when to seek more specialised support such as occupational health.

Main Roles & Responsibilities

  • To provide one-to one support for people struggling with managing their employment and health conditions, with the aim of:
  1. Improving peoples knowledge, skills and confidence in managing their health condition/s,
  2. Empowering and motivating people to set goals to improve their employment and health outcomes,
  3. Encouraging access to services and groups to improve employment and health outcomes.
  • Use a structured framework/model approach to coach individuals across a series of sessions to: identify whats important to them; set personal goals and appropriate steps; build skills and confidence to achieve goals; and use problem-solving to work through challenges.
  • Manage and prioritise a caseload, in accordance with needs, priorities and support required by individuals in the caseload. Along with a strong awareness and understanding of when it is appropriate or necessary to refer people to other health professionals or agencies, because the persons needs are beyond the scope of the role e.g. when there is change in the persons health condition or where unmet clinical support needs are identified.
  • Be proactive in developing strong links with local organisations to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.
  • Engage with employers to support employees with reasonable adjustments and promote support available to them and their employees who may be struggling.
  • Work as part of a multidisciplinary, multi-agency team to support the delivery of the WorkWell Pilot.
  • Understand when it is appropriate or necessary to refer people to other health professionals/agencies.
  • To support patients to access community facilities, groups and community activities/resources at a locality level and understand neighbourhood facilities.
  • To participate in relevant service audits and responsible for capturing and collating data that demonstrates a positive impact on peoples outcomes.
  • To provide regular performance reports (including analysis) in relation any on-going work stream.
  • To attend supervision, case management and steering group meetings as required.
