As a Digital Transformation Lead you will be asked to appraise digital options and technologies for
general practice as they develop/emerge and encourage practices and PCN in
utilising their full functionality to enable improvement and operational
efficiency.
Main duties of the job
Main Role and Responsibilities
Work closely with the PCN Clinical Director &
management team, and staff to understand current service models and processes,
identify challenges, agree priorities for improvement and then support
delivery.
Provide feedback to the management team on progress
of current initiatives, capacity for change, and issues so that plans can be
adapted in response.
Maintain an up to date understanding of all PCN contracts
and service specifications and ensure all relevant legal and contractual
requirements are met and best practice is followed.
Build relationships and facilitate collaboration
between practices, stakeholders and the wider system to support the delivery of
care to patients.
Be a champion for best practice, using data and
analysis to inform and enable innovation and transformation within the PCN.
Review of the PCNs digital maturity, and lead
improvement including optimisation of new technologies to enhance patient
access and improve efficiency within the PCN.

Job responsibilities
PCN Service Development and Achievement
Maintain knowledge and understanding of core
contracts, Direct Enhanced Services (DES), Impact and Investment Fund (IIF) etc
ensuring staff understand their role in achievement of these; and support
provision of training and feedback to ensure targets are met.
Agree appropriate systems for delivery of services
and ensure implementation through robust monitoring and quality reviews.
Working with the PCN Management Team, support
workforce planning for the PCN by considering the future workforce needs in
relation to delivering the PCN vision, priorities, specifications, and growth.
Ensure transformation projects include meaningful
and valued engagement and involvement with users of its services, and that this
is fully embedded in the development of services within the PCN.
Quality Improvement and Data Analysis
Use Business Intelligence, data and local
information to identify opportunities in improving population health and
reducing health inequalities, within the neighbourhood and locality plans,
collaborating on relevant projects with partners.
Support the delivery of projects and write and
present regular performance reports for the PCN Board demonstrating progress,
identifying issues, and making evidence-based recommendations.
Identify potential areas for collaboration across
the PCN practices to gain efficiencies, improve services and deliver high
quality patient care.
Review and regular audit of the utilisation of the
PCNs enhanced access clinical sessions
Other Responsibilities
Attend appropriate PCN meetings as required and
wider meetings as appropriate.
Horizon scan for opportunities, barriers, threats
and changes, assessing these and bringing them to the attention of the PCN
Management Team for consideration.
Undertake any training required to maintain
competency including mandatory training.

- Experienced in delivering multiple programs of IT projects through the entire project lifecycle
- Good understanding of the Digital agenda within the NHS and Social Care or Care Sector
- Experience in using quality improvement methodology/tools
