Job summary

An exciting opportunity within Victoria Primary Care Network (PCN) has opened for an Advanced Clinical Practitioner to become an integral part of a new forward-thinking multidisciplinary team to work within our Frailty Team. This team will support frail and vulnerable patients both in Care Homes and who are in their own homes potentially with multiple co- morbidities and complex long-term conditions.

The ideal candidate will be motivated by change within primary care and help adapt new processes and ways of thinking, leading the team fully utilising advanced clinical practitioner skills and experience. The post holder will be part of a multi-disciplinary team, where you will be able to build up working relationships with patients to help improve quality of life for some of our most vulnerable.

You will help mould a new service that is beneficial to patients and GPs. You will have close working relationships with your colleagues and receive support from management and peers. If you would like to work alongside a team of Practices that are dedicated to improving the quality of life for their patients and help shape a new beginning within primary care this role is for you.

Main duties of the job

We are looking for someone with advanced clinical practitioner skills confident in managing medical complexities and with first-class communications skills. Working across the four GP practices within the PCN, the successful candidate will work closely with GPs, Nurses, Physician Associates, Social Prescribers and Clinical Pharmacists as part of a multi-disciplinary team while also building strong effective working relationships to establish and deliver quality care to our Care Home patients

Job description

Job responsibilities

Play an integral role as part of the MDT caring for the PCNs Care Homes and vulnerable house bound patients. Other duties will include:

Setting up specialist clinics within general practice based on patient cohort

Managing patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

Providing advanced, clinical and specialist nursing practice to patients as required in accordance with clinical based evidence, NICE and the NSF

Requesting pathology services as necessary

Processing and interpreting pathology and other test results as required

Referring patients where medically appropriate and within guidelines

Maintaining accurate clinical records in conjunction with extant legislation

Ensuring read codes are used effectively within clinical system (EMIS)

Maintaining chronic disease registers

Developing, implementing, and embedding health promotion and well-being programmes whilst prioritising health issues and intervening appropriately

Supporting the team in dealing with clinical emergencies where necessary

Recognising, assessing and referring patients presenting with mental health needs

Extended and supplementary prescriber, adhering to extant guidance

Supporting patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

Contributing to PCN targets, complying with local and regional guidance

Liaising with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

Delegating clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual

Supporting the clinical teams with all safeguarding matters, in accordance with local and national policies

Understanding local policies for substance abuse and addictive behaviour, referring patients appropriate
