Job summary
Modern
General Practices have access to a wide range of software tools to assist them
in providing high quality and efficient patient care. Some practices are not
leveraging the maximum possible benefits from these systems and busy practice
staff do not have the time, and in some cases lack the skillset to fully deploy
these systems.
As
the Digital Transformation Manager, you will work with GP practices and wider
Digital Transformation Team to help them to make the best use of their IT
solutions.
Main duties of the job
The Digital
Transformation team will work in and with practices to:
• Benchmark their current use of systems
• Identify opportunities for greater use of systems
• Lead business process change, optimising workflows by using IT
effectively
• Share best practice from other practices and vendors
• Train Viaduct Care and GP practice staff
• Configure systems
• Troubleshoot
• Work with vendors and commissioners to get the best from the systems
• Enhance practice websites to help patients access services
Promoting
excellence in practice and leading system improvements across primary care, the
role encompasses the development and implementation of the infrastructure in
which practices will operate, providing assurance of good practice and
exceptions across the key domains of responsibility and in accordance with
agreed local and national objectives, targets, quality standards, controls and
resource constraints.
As the Digital Transformation Manager you will
plan and project manage the work of the Digital
Transformation team, while also taking an active part of the work itself. You
will also be responsible for reporting on the team’s activity and the
transformation progress of GP practices.

Viaduct Care CIC is Stockport’s GP
Federation and represents its 35 local GP Practices. Covering a patient population of circa 320,000,
practices are split into 7 Primary Care Networks each serving a population of
around 30,000-50,000 patients. Viaduct
Care represents the collective voice and interests of its member practices and
aims to influence and support the design and delivery of major service and
system changes by being a strong and effective partner with other
providers. As part of this we want to
ensure that capacity, capability and sustainability within general practice is
effectively increased so that Stockport is recognised as a great place to work
for those already employed within the primary care sector but also attractive
for those considering a career in this area.

Main Roles
& Responsibilities:

Plan and manage a
programme of work to help GP practices to leverage the maximum benefit from
their IT solutions.
·
To accurately
provide data (reports/verbal updates) within the scope of the Digital
Facilitator workstream as required by senior management and provide advice on
how to interpret the data to inform decisions.
·
Use project management
methodologies to monitor progress of the project across each GP practice and
adjust plans as required.
·
Be responsible for reporting on the team’s activity
and the transformation progress of GP practices.
·
Identify and target areas where
the team’s work can have the greatest benefit for practices.
·
Assess the capacity of available resources when
solving problems and present a range of options when identifying solutions.
·
Act as subject matter expert on specific GP
practice systems and software packages, as required.
· Support the Digital Facilitator team to gain the
skills needed to successfully complete the project.
· Horizon scan to anticipate practice IT needs
and identify opportunities for adding value to service delivery.
·
Lead on business
process change within practices to ensure staff are able to make optimum use of
available technology.
·
Support and train
Viaduct and General Practice staff at all levels so that they become confident
in using new Digital tools.
·
Develop and
deliver internal and external communications to support the project e.g. emails,
newsletters, workshops & reports
·
To communicate
effectively at all levels within General Practice and Viaduct and promote
effective lines of communication with GP practices.

Facilitators to deliver the PCN Digital Facilitator function
- Participate in the
recruitment and retention of staff and assist in the induction of new
staff incorporating the necessary statutory and Viaduct CIC policies and
procedures.
