Job summary
To support people with dementia, and their carers and families in identifying their support needs, offer appropriate solutions and advocate for the families if required.
To support and work with the Herefordshire and Worcestershire Health and Care Trust in delivering the Diadem project in care homes.
To work within care settings to support the understanding of the necessity for person centred support plans and the need for providing suitable and appropriate person-centred support within the care setting.
To promote the use of the Herefordshire and Worcestershire Life Stories Platform as a tool for recording patients life stories for both their families and to increase understanding of the individual for carers.

MAIN RESPONSIBILITIES AND DUTIES
To deliver person centred support to both people with a diagnosis of dementia, their carers and families in a care setting and support care homes to deliver meaningful support for those living with dementia.
To work in partnership with the Herefordshire and Worcestershire Health and Care Trust Diadem project.

To review service users bi-monthly and maintain regular contact with care settings.
To promote the use of the Herefordshire and Worcestershire Life Stories Platform as an effective tool for recording life stories and histories.
To maintain accurate, up to date service user support plans and service user contact record.
To assist in raising public awareness about dementia and the way it affects families and individuals, and to participate in local events such as conferences.
To have a proactive approach in reaching people with dementia, carers and care settings who may not otherwise access our services.
To seek and record service user feedback to understand whether needs are being met, and to plan any necessary adjustments.
To develop and deliver effective systems of working and procedures creating a culture of continuous improvement.
To support the development of KPI reporting, ensuring delivery of the Strategic Plan and a commitment to demonstrating impact across the organisation.
To prioritise Safeguarding our clients and colleagues.
To support person centred evaluation processes in order to inform current and potential service development.
To be willing to travel and work flexibly throughout the charities operational area
To be prepared to work from any of our operational bases as required
To work in accordance with GDPR, Safeguarding, best practice and quality frameworks.
To take responsibility for own personal development and learning. Undertake training as required.
To positively promote and publicise the work of Age UK H&W.
