Job summary

This is an extremely exciting time to join the charity. Over the past three years we have embarked on an exciting period of transformation and growth which has led to an increase in services; the size; and reputation of the organisation.

This position is a fun and rewarding opportunity to work closely with older adults in Sheffield and make a visible difference in their lives. The Project Lead works closely with internal and external colleagues to help older people who have suffered a fall and as a result been hospitalised or is at risk of readmission shortly after discharge.This is a graded intervention with the aim to avoid admission to hospital due to falls.

Older people are more vulnerable and likely to fall, especially if theyhave a long-term health condition.Most falls do not result in serious injury. But there's always a risk that a fall could lead to broken bones,which can mean a stay in hospital, reliance on carers and can causethe person to lose confidence, become withdrawn, and feelas ifthey have lost their independence.

Main duties of the job

Interventions may include but are not limited to:

  • Removing clutter, trailing wires, and frayed carpet
  • Encouragement to use non-slip mats and rugs
  • Making sure all rooms, passages and staircases are well lit
  • Helping to organise the service user's home so that climbing, stretching and bending are kept to a minimum, and to avoid bumping into things
  • Encouragement to wear appropriate footwear and clothing
  • Advice around foot care, trimming your toenails regularly and seeing a GP or podiatrist (foot health professional) about any foot problems
  • Working with them in their home on daily tasks to make sure they are being completed in a safe way and advising on different options
  • Taking service users to the shops or appointments to gain confidence in a safe way
  • Assessing and identifying appropriate equipment
  • Referrals to equipment and adaptations for appropriate mobility and independent living aids - shower chairs, kitchen trolley, Mowbray toilet frame, grab rails etc
  • Helping to arrange eye tests
  • Discussion with service user and GP about medication which may put them at risk of a fall
  • Educating the service user on the safe use of equipment

This will be a person-centred, holistic approach. The service users needs and wishes will be taken into consideration when their plan is created. Referrals would be taken from NGH, RHH, Weston Park, STIT, Active Recovery, Front Door Response Team, Home First, Social Services and GPs

Duties and Responsibilities

  • To create risk assessments in line with NHS and PH England guidelines and within the remit of SCCCC
  • To take ownership of the implementation of the Reablement Project
  • Working in a client-centred approach, assess service users and their environments (including risk assessments)
  • To create and implement person-centred plans to allow the service user to regain and retain confidence to mobilise within their community and prevent a falls related admission to hospital
  • To ensure service users safety is prioritised, and alert partner organisations when there are high levels of risk
  • To facilitate collaborative working with all agencies across the health and social care system including statutory, voluntary or independent sectors to implement sustainable evidence-based practice to aid recovery from and prevent falls
  • To produce promotional material and organise, attend and present at publicity events to raise awareness of falls prevention and Reablement
  • To attend multi-disciplinary team meetings, working with colleagues to identify individuals suitable for referrals to the scheme
  • To induct, supervise, develop and support a team of volunteers (and from time to time any SCCCC staff as directed by the Operational Lead) in order to ensure the efficient and effective running of the Reablement Project
  • To put in place systems and procedures to record client information in accordance with GDPR and privacy policies and to capture and evaluate service monitoring requirements such as outputs, outcomes, and quality
  • To proactively engage in supervision and personal development
  • To collate data and extrapolate information to analyse patient needs, success rate in preventing readmission and identify possible future services
  • To proactively market and promote the range of services offered by SCCCC and its partners where appropriate, to support individuals to retain, regain and develop the skills to manage their lives and environment
  • Ensure compliance with all relevant SCCCC policies and procedures and Quality standards and adherence to good practice guidance
  • To develop and sustain effective relationships with service users, their carers and families, volunteers, SCCCC colleagues, NHS colleagues, other partners and the wider community in order to promote the safety, independence, health, well-being and personal development of older people in Sheffield
  • To provide regular reports and information to the Operational Team Lead, Multi-Agency Steering Group or other nominated colleague in order to meet the reporting needs of all funders and commissioners in a timely manner
  • Other duties as directed by the Operational Team Lead which are consistent and commensurate with the responsibilities of the post and the needs and development of the Reablement Project
