Step Together is a trailblazing service that offers a psychological, adversity and trauma (PAT) informed approach to those rough sleeping/at risk of homelessness with multiple, complex needs (mental health, forensic history, substance use) in Somerset. We advocate for system change, creatively thinking with other agencies to develop innovative ways to overcome barriers to engagement, turning these into opportunities to steer positive trauma-informed system change.
Main duties of the job
Working to the Recovery Coach Model principles, you will develop strengths-based actions orientated around the Outcomes Star. You will engage and support your own caseload of clients with complex needs in leading their journeys, and maximising their choice and control over the support they receive by developing a responsible and individual network of support.
You will have an understanding of issues relating to homelessness and mental health, and be able to build and sustain relationships with clients. You will have a commitment to promoting independence and supporting vulnerable people in developing move on and recovery plans.
We work in a recovery focused, psychological, adversity and trauma (PAT) informed way in all aspects of the teams practice using relevant interventions, including Assessments, Support Planning, Risk Assessing, Brief Interventions and Solution Focused Therapy.
