Job summary

The Community Rehabilitation Service is an award winning service provided by Second Step in partnership with Avon and Wiltshire Mental Health Partnership NHS Trust and Missing Link. Shaped by people who have used mental health services, the Community Rehabilitation Service gives people the support they need to achieve their hopes and goals. We are passionate about recovery and the service is Trauma informed. The post will be based at Second Step office at Pennywell Road, Bristol.

Main duties of the job

You will work as part of a wider multi-disciplinary team, having an oversight of the team caseload and leading the work to ensure that all service users are supported to access the required physical health screenings. You will have a key role in delivering specialist physical health and wellbeing interventions, ensuring that services are tailored to individuals cultural and social needs.

Ensure that all services are delivered in a warm and welcoming way, demonstrating ambition and hope for all service users. Build trusting and collaborative relationships with service users. Carry out role adhering to Second Steps policies, procedures, values, code of conduct and current legislation using a Recovery orientated, psychologically informed, equalities based approach Contribute to the co-production of Personal Wellbeing Plans, crisis plans and other self-management tools, in partnership with service users and their Key-workers in line with best practice. Provide support, referring and liaising effectively with all professionals, agencies and other parties involved in service users support in order to deliver the agreed plan. Work closely with colleagues within the service and build strong partnership working with other agencies such as Primary care, Health and Social Care, Voluntary and Community Service. Essential

  • Experience of supervising staff, students or volunteers.
  • Two years significant experience of working with people with mental health needs including needs assessments and the support planning process.
  • Experience of working with people with multiple needs i.e. mental health, homelessness, alcohol/drug dependency.
  • Experience of providing practical and emotional support to enable the client to take up the responsibilities of their own tenancy and settle and reintegrate into the community.

Desirable

  • Proven experience of working within a housing and support agency
  • Proven experience of mental health issues and/or services, either as a user, carer or supporter
