Job summary

Would you like to work in an exciting new mental health role in primary care? New Mental Health and Wellbeing Integrated Network Team (MINT) mental health hubs have been set up in communities across Bristol, North Somerset and South Gloucestershire to change the way we support people struggling with their mental health. MINT hubs are at the forefront of a new and exciting system of integrated mental health support set up across England by the NHS. MINT stands for Mental health and wellbeing Integrated Network Teams.

Main duties of the job

Youll be joining our MINT teams of skilled mental health professionals and working alongside colleagues from AWP, social care, GPs and voluntary care staff. Together well be joining up services to help better support people with moderate to high mental health needs. Your role in the MINT team will focus on those vital early support conversations with clients.

Building trusting and collaborative relationships with clients - demonstrating hope and courage to help maximise their independence; providing advocacy when needed Completing individual assessments via initial support conversations to inform clients recovery plan and direction of support Supporting clients to use self-management tools such as a Wellness Recovery Action Plan Being trained to use a range of skilled interventions and approaches, such as motivational interviewing, active listening and solution-focused support Helping clients navigate the system to find the support they need via voluntary sector organisations or referrals to statutory services like drug and alcohol support

  • Experience working with people with mental health needs, or people with

  • Proven experience of working with people with multiple needs (e.g. mental
  • health, homelessness, alcohol/drug dependency long term conditions)
  • Proven experience of carrying out needs and/or risk assessments and the
  • support planning process
  • Experience of working with a number of individuals with competing needs
  • and priorities

  • Proven experience or knowledge of mental health issues and/or services,
  • either as a user, carer, or supporter
