Job summary
We are recruiting a part time Recovery and Wellbeing Worker to join our Step Down Team in Wells to support individuals leaving hospital to transition back to independence. The Step Down Service will work with clients on their journey from acute services to their own accommodation, coproducing strength-based, individual recovery plans that provide strategies to; better manage their mental health, identify warning signs, build resilience and nurture positive relationships. From the moment someone moves in, the focus will be on life after move on and building practical living skills to support independence and wellbeing.
Main duties of the job
Support will be provided through 1:1 and group sessions. Beyond their stay at the Step Down house, we will engage with local partners and our wider Open Mental Health VCSE Alliance (OMHVA) support offer to ensure sustained positive outcomes.
