Job summary

We are seeking a motivated individual who can demonstrate the skills required to develop trusting relationships and support individuals experiencing multiple disadvantages and facing significant systemic barriers to accessing and engaging with support and healthcare. The successful candidates will be working within the Health Link Team alongside partner teams within the Homeless Health Service as well as a wide range of external agencies coordinating tailored support, identifying creative solutions, and empowering individuals to access and engage with health services more independently.

Main duties of the job

The Health Link team works with people who are homeless, at risk of repeat homelessness or who are vulnerably housed in order to improve their health, recovery and maximise engagement with healthcare and related services. The team specialises in working with people who experience chronic, complex health needs and multiple disadvantages, and who face systemic challenges which impact on engagement with health services. Health Link works with individuals who are frequent attenders of A&E services.

Provide assessments of the health related support needs of vulnerable individuals who are homeless or at risk of homelessness, working in tandem with other members of the Homeless Health Service team. Provide a signposting, advocacy and referral service to other relevant agencies, including follow up liaison to facilitate engagement and progress

Experience

Essential

  • Two years significant experience of working with homeless people.
  • Experience of working with people with multiple needs i.e. mental health and alcohol/drug dependency.
  • Experience of providing housing or health related support, including risk assessment and support planning

Desirable

  • Proven experience of providing housing or health related support
  • Proven experience of mental health issues and/or services, either as a user, carer or supporter
