Main duties of the job

Being a Band 5 Registered Nurse at St Wilfrid's

This role provides a safe, effective, efficient care environment for patients, relatives and carers. As a registered nurse you will provide pro-active and skilled nursing care to patients in our inpatient Unit in need of specialist palliative end of life care, undertaking nursing duties and holistic assessments, planning and co-ordination of a patients needs with programmes of care that are supportive, compassionate and person-centred.

Job responsibilities

JOB PURPOSE

To provide a safe, effective, efficient care environment for patients, relatives and carers

Provide pro-active and skilled nursing care to patients in need of specialist palliative end of life care, undertaking nursing duties and holistic assessments, planning and co-ordination of a patients c needs with programmes of care that are supportive, compassionate and person-centred.

The post holder will be involved in supporting carers and loved ones pre and post bereavement.

To mentor and supervise pre and post registration staff without direct supervision.

In the absence of the Clinical Lead or Senior Staff Nurses play a key role in leading and coordinating a shift

KEY ACCOUNTABILITIES & RESPONSIBILITIES

Clinical Practice

Provide skilled end of life care, symptom control and long-term conditions management, based on principles of best practice and clinical evidence.

To carry out the full range of registered nurse duties, including assessment, planning, implementation and evaluation of the patients condition

Provide specific nursing skills such as the administration of medication via syringe drivers and intravenous therapy including blood transfusions, PEG and NGT management, urinary catheterisation and bowel management, oxygen therapy management and routine patient monitoring.

To deliver individualised and person-centred care plans that provide monitoring and promotion of self-care to prevent unnecessary hospital admissions or promote timely discharge.

Under supervision take responsibility for the continued holistic health needs assessment ensuring continuity following referral from acute care, GPs and other agencies.

Act as the patients advocate to facilitate the patients own choices with regard to nursing care, promoting informed patient preferences.

Recognise the changing needs of a patient at the end of their life (and those close to them) and know how to respond.

Provide information and support at end of life, including help with the practical, emotional and financial impacts of supporting someone in times that can be stressful and emotionally demanding.

Provide both practical and emotional support to those close to the patient who is facing an anticipated loss of a loved one.

Be alert to the needs of vulnerable adults and taking appropriate action when required, escalating any safeguarding concerns.

Provide care to people with long term conditions undertaking reviews in accordance with the individual patient care plan and with support of more experienced colleagues as required

Have a raised awareness of any ethical questions that might arise when providing support to patients and loved ones and how to address these

Safe handling of body fluids and contaminated sharps in accordance to Hospice policy.

Undertake appropriate risk assessments.

Management and Leadership

Organise and prioritise own work without direct supervision, reporting any issues, concerns or personal limitations.

Supervise and co-ordinate the work of junior staff, including student nurses, on a shift by shift basis.

Apply and promote equal opportunities in practice considering the individual needs of patients, their loved ones and colleagues

Maintain accurate, comprehensive and timely records of care within Hospice and NMC guidelines.

Initiate referrals or SystmOne taskings, to ensure adequate care and support to patients and their loved ones.

Ensure effective communication with wider healthcare services such as completing Continuing Health Care funding applications and writing discharge letters.

Take appropriate action in regard to any safeguarding concerns, accidents or incidents to patient, staff or visitors within the scope of the post holders responsibility and in line with Hospice guidelines.

Support medicines management and reviews as directed, teaching and supporting carers to give sub cutaneous medications where appropriate.

Attend and participate in MDT meetings as well as team, locality and other relevant meetings

Communication

Maintain positive and effective communication and relationships with patients, their loved ones and all other professionals to enhance a dignified and high standard of care provision

Understanding of the need for effective communication in discussing end of life care with individuals reaching the end of life and those close to them

Give information in a sensitive and planned way to allow time and privacy. Language should be clear and understand- able, avoiding jargon. Written information may be given to family to take away.

Communicate complex and sensitive patient information to colleagues within the primary healthcare team and to other multi-disciplinary team members.

At all times have an awareness of potential barriers in communication, understanding how to overcome these barriers, to maintain effective communication and equity of service provision

Education and Training

Attend relevant education and training programmes to maintain professional development and meet NMC and St Wilfrids requirements to ensure revalidation is met.

Reflect on practice regularly and plan professional development in order to achieve growth and development.

Access clinical and managerial supervision to enhance reflection on practice.

Actively involved in promoting evidence-based practice

Personal and Professional Development
