Job summary

We have a brilliant opportunity for two Senior Clinical Case Managers to join our brand new Complex Transition Support Service!

The service will work closely with North East and North Cumbria (NENC) Integrated Care Board (IBC) Complex Case Management team. The overall aim for CTSS is to support the discharge of people from acute secondary care inpatient beds / private hospital beds into alternative community settings. CTSS will also assist in the repatriation of people back to the NENC region for those currently residing out of area. The service will provide assessments, discharge planning support and 12 weeks post transition support to settle people into their new accommodation.

We are keen to here from Registered Mental Health Nurses (RMN), Occupational Therapists (OT) and Social Workers who have experience working with complex mental health needs, autism and learning disabilities.

Main duties of the job

  • Manage the needs of service users, carers and partner services to ensure delivery (inclusive of all required resident records) meets all relevant quality standards and is person centred, outcomes focused and protective of individual service users dignity and safety.
  • Ensure effective management of the caseload of either CTSS Team A or Team B, striving to find solutions that support timely and safe transition of service users their identified accommodations.
  • Directly line manage up to 2 Clinical Case Managers.
  • Manage the service users and team outputs using audits, standards and investigations, to ensure a high quality and consistent service is delivered to service users, cares and partners.
  • Carry out assessments and develop discharge plans when required.
  • Oversee transition support plans that will be implemented with the receiving service and create / deliver relevant training to partners that will support effective transition and reduce likelihood of relapse and/or readmission to hospital.
  • Work with ICB Case Management team on caseload management.
  • Support funding panel decision making by providing timely case notes and information to ICB Case Managers / Local Autority Placement Coordinators.
  • Horizon scan to ensure that service users accessing CTSS are receiving high quality support and ensure evidence based best practice is in place.
  • Deputise for CTSS Specialist Service Lead when required.
