Job summary

We have an exciting opportunity for a Specialist Service Lead to join our brand new Complex Community Transition Service!

The service will work closely with North East and North Cumbria (NENC) Integrated Care Board (IBC) Complex Case Management team. The overall aim for CTSS is to support the discharge of people from acute secondary care inpatient beds / private hospital beds into alternative community settings. CTSS will also assist in the repatriation of people back to the NENC region for those currently residing out of area. The service will provide assessments, discharge planning support and 12 weeks post transition support to settle people into their new accommodation.

We are keen to here from Registered Mental Health Nurses (RMN), Occupational Therapists (OT) and Social Workers who have experience working with complex mental health needs, autism and learning disabilities.

Main duties of the job

  • Managing day to day operations of the CTSS team
  • Direct Line Management of 2 x Senior Clinical Case Managers
  • Working with the ICB complex case team to agree caseload and new referrals and maintain effective communication with the team
  • Monitoring effectiveness of services and evaluating service user outcomes.
  • Guidance on navigating NHS & Local Authority funding streams to ensure timely agreement of support packages
  • Ensuring effective clinical governance approaches are adopted within the team and adhere to Everyturn policies
  • Supporting Head of Specialist Residential Services in contract meetings with the ICB around performance and governance, including activity reporting
  • Ensure that national guidance and best practice around supporting transition from acute services, private providers and out of area placements are adopted and remain aware of strategic focus areas specifically around Mental Health Urgent and Emergency Care Pathways.
  • Represent Everyturn within ICB strategic boards, including the Inpatient Quality Transformation programme, and any other boards/committees relevant to the effective delivery of CTSS.
  • Joint working with Mental Health trusts, private providers, community services and housing providers to ensure effective flow of individuals through the system and fostering an environment of continuous change and service development
  • Maintaining positive relationships with Bed Management teams at NHS Trusts to ensure integrated working practices.

  • Significant experience of working with clients who may present as highly emotive and at times present as aggressive or difficult.
