Job responsibilities
- To help create a
welcoming, caring, safe and supportive environment for patients and
families.
- Work enthusiastically and
creatively, maximising the patient experience when attending Hub services.
- To assist in
planning, implementing and evaluating care in conjunction with the
patient/family.
- Assist in maximising and maintaining
the functional and social independence of patients through a range of low-level
therapeutic interventions under supervision including:
Handling
and positioning
Passive
and active exercise programmes
Breathlessness
management programmes
Anxiety
management programmes
- Support with the delivery of Group
works e.g. exercise and breathlessness groups.
- Report to senior staff any concerns,
changes or deterioration of the patients condition as the change occurs.
- Carry out patient assessments within
the agreed competency level and under the guidance of an Occupational
Therapist/Physiotherapist.
- Work as part of the
multidisciplinary team and participate in Hub team meetings including
handover and planning meetings.
- Liaise with other professionals
working within Havens Hospices, primary and secondary care settings.
- Have a good knowledge and
understanding of manual handling, and the use of equipment to support
other staff members and ensure the patient's safety.
- Due to the unpredictable nature of
referrals to the service, and the needs of the patients and relatives, be
flexible in working patterns.
- To empower and enable the family and
friends to continue caring for the patient and support them as
appropriate.
- The post holder is legally required
to have regard to the Mental Capacity Act 2005 Code of Practice and
Deprivation of Liberty Safeguards.
- Use of
professional judgement to analyse complex information and make decisions
in the best interest of the family
- Observes
personal duty of care concerning equipment and resources used in course of
work.
- To
manage own well-being with the support provided due to the frequent
exposure to patients and families in distress
- Ability
to work with occasional exposure to unpleasant working conditions
Communication
- Communicates effectively with members of the
multidisciplinary team, District Nurse, GP, Social Services, and other
organisations
- Communicate sensitively with patients and
families who are often emotional due to distressing situations
- Providing and receiving complex and sensitive
information. Using persuasive, motivational, negotiating, training,
empathic or reassurance skills are required. This may be because agreement
or cooperation is required or because there are barriers to understanding

Adheres to the services standard
operating and alerts the manager of any operational conflicts that require
amending/updating.

Administration

SystmOne and records in the patients home following Hospice policy,
including other specialist notes e.g. Integrated Care Pathway, and PPC
documents.
- To ensure confidentiality of
information at all times, in both written and verbal form.
- To take part in any audit of care
services.

- To participate in annual appraisals.
- To identify own ongoing educational
needs and discuss with the Director to achieve those development needs.
- To be prepared to develop skills and
competencies that are required to meet the demands of the role as it
evolves overtime period.
