Job summary
We have an exciting opportunity for an experienced health care assistant to join our Day Hospice team.
We are looking for a highly driven, energetic and creative individual who possesses excellent personal care skills and who can provide interesting and stimulating activities for our Day Hospice patients.
Applicants must have excellent communication skills, be motivated and innovative and possess the minimum of a Level 3 Diploma in Health and Social Care or the equivalent.
2 posts available of between 1 & 3 days a week Monday to Thursday (both posts will be required to work Wednesdays), working 8.30am to 4.30pm and some flexibility is required in respect of the days worked to help meet the needs of the service.
You may be required to work in any of our Day Hospices, including Blackett Avenue, Ystrad Mynach.
Main duties of the job
The
HCA will work independently providing safe, competent, and ethical client care
under the guidance of registered healthcare professionals, in the delivery of
palliative care to patients and their families, utilising protocols and
procedures.

Key Areas of Responsibility:
- To
carry out tasks involving direct patient care and support to patients and
families
- To
undertake assessments of patient/carer needs
- Co-ordinate
patient transport and document accordingly liaising with ambulance transport
and taxi firms.
- Hostess
duties and assist patients with meals, nutritional needs.
- Be
willing to transport patients using company vehicles when required.
- To
act within the limits of your competence and authority
- To
support and direct clinical care assistant volunteers
- Contribute
to the effectiveness of teams by participating in inter-disciplinary team
working to support individuals.
Clinical:
- Communicate effectively with patients, carers and professionals
- Support individuals who are distressed
- Assess an individuals needs arising from their health and social status
- Undertake personal hygiene for individuals unable to care for themselves
- Prepare individuals for health and social care actions activities through the appropriate moving and positioning of individuals.
- Prepare and manage environments and resources for use in health and social care activities
- Promote participation in agreed therapeutic group activities
- Enable patients to develop the knowledge and skills to manage their illness/social situation
- Assist individuals in undertaking activities and enable them to choose and participate in activities that are meaningful to them.
- Work in collaboration with family/carers to enable them to support individuals
- Deliver and collect clinical items if required.
Must have a clean drivers licence and access to a car
