Main duties of the job

Our Hospice Navigators support patients and carers to navigate the clinical services available to them at Farleigh and provide administrative support to our multidisciplinary clinical locality teams.

Job description

Job responsibilities

As a Navigator you will work within a multi-disciplinary team (MDT) to support patients and carers to navigate the clinical services within the Hospice. By being up to date with all aspects of the Locality Care Team, the Navigator will be able to direct patients based on their initial presenting problem to the most appropriate member of the team. This role will be focused on providing general information and support about the hospice and the Locality Team services, to enable people to navigate the health and social care system and make choices that are best for them

  • A key aspect of the role involves daily communication with patients, relatives, carers, and other health and social care professionals. The information and nature of the communication required is sensitive due to the nature of life limiting illness. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity
  • The post holder will utilise their own knowledge of Hospice care to direct the patient to the most appropriate clinical resource within the Locality Care Team, based on their presenting need reducing the impact on clinical staff by signposting individuals whose needs cannot be met within the current hospice services
  • Identifying and directing these callers to community or voluntary services that may be able to meet these needs as required
  • The role requires use of judgment in responding to the needs of individuals. The level of judgment required relates to identifying the complexity of the situation, providing appropriate advice and escalating to the registered practitioner where appropriate
  • The post holder will report any concerns regarding individual patients to the Locality Care Team Manager or clinical member of the team
  • The Post holder will demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or an emergency situation, alerting the clinical team members immediately
  • To support and implement any streamlined locality processes and services by embedding quality improvement methodology including clear outcome measures to produce efficient and highly performing community services
  • To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, CCG, health and social care partners, businesses and voluntary organisations
  • To undertake both routine admin tasks and specially identified tasks as per training and assessed as competent
  • To carry out administration duties required by the role including filing, report writing, documenting decisions made
  • To understand that there will be some distressing situations and when to seek appropriate support/advice
