Job summary

Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region.Our team at Farleigh has grown and evolved to meet the changings needs of the community we serve and we are looking for a highly motivated Care Coordinator to lead the procurement of nursing home placements or domiciliary care for patients thought to be in the last three months of life.

Farleigh Hospice is the leading specialist palliative care provider in mid Essex, providing specialist support to patients and their families in both the community via our multidisciplinary locality care teams, and our inpatient unit.

Main duties of the job

We are looking for a skilled Coordinator to develop relationships with external organisations, manage timescales sensitively and liaise with patients and relatives to ensure that the patient has access to the most appropriate care environment for their own specific needs.

Job description

Job responsibilities

  • To co-ordinate the procurement of Nursing Home and Domiciliary Care for Mid Essex patients at the end of their lives (last 12 weeks)
  • To be responsible for data collection for all aspects of placements utilising SystmOne records and computerised databases
  • To work closely with all clinical teams at Farleigh Hospice and other provider organisations to deliver the above service and to escalate clinical concerns and sourcing difficulties to the Rapid Access to Discharge Manager
  • To support the Rapid Assessment and Discharge Team with administrative support as required Through telephone, virtual and face-to-face discussions, provide information to patients with palliative and end of life care needs, supporting their families and carers, liaising with clinical professionals and referring on as necessary to appropriate health and social care partners
  • To support the Hospice in building strong relationships with external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, ICB, health and social care partners, businesses and voluntary organisations
  • To reconcile the invoices against care delivered to ensure accurate balances and raise with care providers if there any reconciliation discrepancies
